肥胖对脑出血患者功能结局的影响

Effect of obesity on functional outcome in patients with intracerebral hemorrhage

目的:探讨肥胖对脑出血功能结局的影响。方法 :回顾性分析300例急性脑出血患者的资料,将体质量指数(body mass index,BMI)>28 kg/m2者设为肥胖组,余设为对照组。比较两组人口统计学和临床特征,进行多变量Logistic逐步回归分析肥胖对脑出血功能结局影响的独立因素,秩和检验分析对预后的影响。结果:300例患者中肥胖组54例,对照组246例。肥胖组患者年龄、男性、吸烟、饮酒、糖尿病史、肥胖家族史、三酰甘油、BMI和腰臀比(waist to hip ratio,WHR)等危险因素与对照组相比,差异有统计学意义(P<0.05);多因素回归分析示三酰甘油等是肥胖脑出血患者预后的独立预测因素;秩和检验示,年龄、三酰甘油、BMI和WHR与肥胖脑出血预后密切相关(P<0.001)。结论:肥胖与脑出血患者功能结局不良密切有关,需加强对肥胖的防治。

Objective： To investigate the effect of obesity on the functional outcome of intracerebral hemorrhage. Methods：The data of 300 patients with acute cerebral hemorrhage were analyzed retrospectively. According to the body mass index（BMI）28 kg/m^2 were divided into obesity group or control group. To compare the two groups of demographic and clinical characteristics; to analyze the influence of obesity on the functional outcome of cerebral hemorrhage by multiple Logistic stepwise regression analysis; and the influence of the rank sum test on the prognosis was analyzed. Results： 300 cases of obesity in 54 cases, 246 cases in the control group. The risk factor ratio of age, male, smoking, drinking, diabetes, family history of obesity, triglyceride, BMI and waist to hip ratio（WHR）, risk factors in the male obesity group was significantly higher than the control group（P〈0.05）; multivariate regression analysis showed that triglycerides were independent predictors of obesity in patients with cerebral hemorrhage prognosis; rank and the test showed that age, triglyceride, BMI and WHR and obesity were closely related to the prognosis of cerebral hemorrhage（P〈0.001）. Conclusion： Obesity is closely related to the functional outcome of patients with cerebral hemorrhage, and it is necessary to strengthen the prevention and treatment of obesity.
