后路椎弓根内固定联合伤椎植骨在老年胸腰段脊柱骨折患者中的应用价值研究

Posterior Pedicle Screw Fixation Combined With Vertebral Bone Graft in Elderly Patients With Thoracolumbar Spinal Fractures

目的探究后路椎弓根内固定联合伤椎植骨在老年胸腰段脊柱骨折患者中的应用价值。方法选取2014年4月-2016年9月在我院进行治疗的老年胸腰段脊柱骨折患者84例,按随机数表法分为两组,各42例。对照组实施单纯椎体成形术治疗,观察组使用后路椎弓根内固定联合椎体成形术治疗,比较两组相关疗效指标及并发症情况。结果术后,观察组疼痛视觉模拟评分法（VAS）评分、Cobby角、椎体前缘压缩率及椎管狭窄率较对照组改善,差异有统计学意义（P〈0.05）;观察组并发症发生率较对照组降低,差异有统计学意义（P〈0.05）。结论后路椎弓根内固定联合伤椎植骨可提高老年胸腰段脊柱骨折患者的治疗效果,降低术后并发症发生率,促进患者功能恢复。

Objective To investigate the posterior pedicle screw fixation combined with vertebral bone graft in elderly patients with thoracolumbar spine fractures. Methods 84 elderly patients with thoracolumbar spine fracture treated in our hospital from April 2014 to September 2016 were divided into two groups according to the random number method. The control group was treated with simple vertebroplasty. The observation group was treated by posterior pedicle screw fixation and vertebroplasty. The relative curative index and complication were compared between the two groups. Results The postoperative visual analogue scale（VAS）score,Cobby angle,compression rate of vertebral anterior margin and stenosis of spinal canal were significantly improved in the observation group,the difference was statistically significant（P〈0.05）. The incidence of complications in the observation group was lower than that in the control group,the difference was statistically significant（P〈0.05）. Conclusion Posterior pedicle screw fixation combined with vertebral bone graft can improve the therapeutic effect of thoracolumbar spine fracture in elderly patients,reduce the incidence of postoperative complications and promote functional recovery.
