前哨淋巴结活检替代腋淋巴结清扫在乳腺癌治疗中的应用价值研究

Study on the Application Value of Sentinel Lymph Node Biopsy Instead of Axillary Lymph Node Dissection in Breast Cancer

目的探讨前哨淋巴结活检替代腋淋巴结清扫在乳腺癌治疗中的应用价值。方法选择2015年5月-2016年6月就诊于我院的乳腺癌患者90例,根据随机数表法将其分为两组,各45例。对照组行改良根治术治疗,观察组采用前哨淋巴结活检治疗。比较两组术后上肢功能（肩关节功能评分、外展度）及上臂臂围差、并发症发生率及住院情况。结果两组术前肩关节功能评分、外展度及上臂臂围差相比,差异均无统计学意义（P〉0.05）;术后观察组肩关节功能评分明显高于对照组,肩关节外展度明显大于对照组,上臂臂围差明显小于对照组,差异有统计学意义（P〈0.05）;观察组并发症发生率明显低于对照组（9.52%〈28.89%）,差异有统计学意义（P〈0.05）;观察组住院情况明显优于对照组,差异有统计学意义（P〈0.05）。结论前哨淋巴结活检替代腋淋巴结清扫有助于减轻乳腺癌患者机体创伤,减少并发症的发生,促进恢复,提高生活质量,值得运用。

Objective To investigate the application value of sentinel lymph node biopsy（SLNB）instead of axillary lymph node dissection（ALND） in breast cancer. Methods 90 patients with breast cancer admitted into our hospital were divided into two groups by random number table from May 2015 to June 2016,45 cases in each. Control group took modified radical operation,observation group took SLNB. Upper limb function after operation（function scores of shoulder joint,abduction degree）and upper arm circumference,incidence of complications and hospitalization condition of two groups were compared. Results Before surgery,there was no statistical difference in the function scores of shoulder joint,abduction degreeand upper arm circumference compared between the two groups（P〉0.05）. After surgery,the function score of shoulder joint in observation group was significantly higher than control group,abduction degree of shoulder joint was significantly larger than control group,and upper arm circumference was significantly smaller than control group（P〈0.05）. The incidence of complications in observation group was significantly lower than control group（9.52% 28.89%）（P〈0.05）. The hospitalization condition of observation group was significantly better than control group（P〈0.05）. Conclusion SLNB instead of ALND facilitates to reduce body trauma of breast cancer patients,decrease the incidence of complications,promote recovery and increase quality of life,which is worthy of application.
