输血治疗老年慢性心衰合并中度贫血40例临床观察

Clinical observation on 40 cases of elderly patients with chronic heart failure combined with moderate anemia treated by blood transfusion

目的观察输血治疗老年慢性心衰合并中度贫血的临床疗效及其对血红蛋白的影响。方法将40例慢性心衰合并中度贫血的患者随机分为对照组和观察组,各20例,对照组给予慢性心衰的标准化治疗,包括利尿剂、ACEI或ARB、β阻滞剂、醛固酮受体拮抗剂等,观察组在慢性心衰的标准化治疗的基础上给予输血400 m L,观察患者的临床疗效及随访3个月后的血红蛋白变化情况。结果观察组临床疗效明显,住院时间更短,与对照组比较差异有统计学意义(P<0.05),且不良反应少;随访3个月后观察组血红蛋白变化与治疗前比较差异有统计学意义(P<0.05),与治疗对照组比较差异有统计学意义(P<0.05)。结论输血治疗老年慢性心衰合并中度贫血可提高临床疗效,更好地改善心功能,减少住院时间,延缓血红蛋白的进一步下降。

Objective To observe the clinical efficacy of blood transfusion in the treatment of elderly patients with chronic heart failure combined with moderate anemia and its impact on hemoglobin. Methods 40 cases of chronic heart failure patients with moderate anaemia were randomly divided into control group and observation group, 20 cases in each group. The control group gives the standardized treatment which the chronic heart fades, including Diuretic, ACEI or ARB, Beta Blockers, Aldosterone antagonists and so on. On the basis of the standardized treatment of chronic heart failure, the observation group lost 400 m Lof blood to the patients and observe the clinical curative effect of patients and the changes of hemoglobin in the next three months. Results The clinical curative effect of observation group was obviously, shorter hospitalization time, compared with the control group was statistically significant difference（P〈0.05）, and have less adverse reaction.After 3 month of follow-up, there was significant difference between the observation group and the change of hemoglobin（P〈0.05）,Compared with the control group, the difference was statistically significant（P〈0.05）.Conclusion Blood transfusion treatment of elderly patients with chronic heart failure complicated with moderate anemia can improve the clinical efficacy, improve the cardiac function, reduce hospitalization time, and delay the decrease of hemoglobin.
