尿胰岛素生长因子结合蛋白7在急性肾损伤诊断中的意义 被引量:2

摘要 目的:探讨尿胰岛素生长因子结合蛋白7(IGFBP7)在急性肾损伤(AKI)早期临床诊断中的意义。方法:选取AKI患者50例为A组,慢性肾脏病患者(CKD)30例为B组,健康体检者30例为C组,检测并比较3组患者确诊时或24 h内尿IGFBP7、血清胱抑素C(sCysC)水平;运用受试者工作特征(ROC)曲线及曲线下面积(AUC)评价尿IGFBP7和血sCysC水平在AKI早期诊断中的意义。结果:①与正常组和CKD组相比,AKI组的尿IGFBP7水平随AKI各期进展而显著升高(P<0.05);②血清sCysC水平,AKI组较正常组显著升高(P<0.05);较CKD组稍升高(P>0.05);随疾病进展AKI组血清sCysC水平逐渐升高,各期之间差异显著(P<0.05);③AKI患者尿IGFBP7水平与血清sCys C及SCr水平成正相关(r分别为0.782或0.884,均P<0.05);④尿IGFBP7水平、血清sCysC水平诊断AKI的AUC分别为0.889、0.877。结论:尿IGFBP7水平在AKI诊断中具有良好的敏感性。
