摘要
目的探讨白细胞介素-31(IL-31)和白细胞介素-33(IL-33)水平变化在子宫内膜癌患者中的临床意义。方法选取子宫内膜癌患者92例为观察组,选取子宫内膜不典型增生以及子宫内膜复杂性增生患者56例为癌前病变组,选取健康体检的志愿者60例为健康对照组,测定3组患者的IL-31和IL-33水平,同时采用免疫发光法测定肿瘤标志物癌胚抗原(CEA)、CA-125、CA19-9水平,分析比较各组患者IL-31、IL-33水平变化及对子宫内膜癌的诊断价值。结果观察组IL-31、IL-33水平明显高于癌前病变组和健康对照组(P﹤0.01);Ⅲ~Ⅳ期子宫内膜癌患者的IL-31、IL-33水平明显高于Ⅰ~Ⅱ期患者(P﹤0.01);ROC曲线分析显示,IL-31的诊断阈值为113.10 pg/ml,AUC为0.973;IL-33的诊断阈值为98.42 pg/ml,AUC为0.929;IL-31和IL-33对子宫内膜癌诊断的敏感度和特异度均高于CEA、CA-125和CA19-9。结论 IL-31和IL-33水平升高在子宫内膜癌的发生发展中发挥着重要作用,IL-31和IL-33对子宫内膜癌具有重要的诊断价值。
Objective To investigate the clinical significance of interleukin-31 and interleukin - 33 levels in patients with endometrial carcinoma. Methods 92 cases were collected in our hospital from January 2012 to January 2016 as the research group, and 60 cases of healthy volunteers in the same period as the control group, Fasting venous blood of all the subjects were collected, IL-31 and IL-33 levels were detected by ELISA and tumor markers (CEA, CA-125, CA19-9) were detected by immune chemiluminescence. IL-31 and IL-33 levels and diagnostic value for endometrial cancer were analyzed.Results IL-31 level in observation group (165.60 ± 30.02) pg/ml was significantly higher than the control group(77.20 ± 20.85) pg / ml (t=17.516, P= 0.000); IL-33 levels in the observation group (108.80 ± 7.36) pg/ml was significantly higher than the control group (74.28 ± 21.90) pg/ml (t= 9.246, P=0.000). IL-31 levels in III and IV of endometrial cancer patients was higher than I and II patients, Patients with endometrial cancer, clinical stage higher or higher TNM stage has the higher IL-33 levels (P〈0.05). ROC curve analysis showed that IL-31 diagnostic threshold value of 113.10 pg/ml (sensitivity 92.68%, specificity 94.87%), AUC was 0.973 (95% CI: 0.945-0.998, P〈0.01); IL-33 diagnostic threshold 98.42 pg/ml (sensitivity of 88.64% and a specificity of 97.22%), AUC was 0.929 (95% CI: 0.860-0.998, P〈0.01). Multiple indexes diagnostic test analysis, diagnostic sensitivity and specificity of IL-31 combined with IL-33 endometrial cancer were significantly higher than CEA, CA-125 and CA19-9. Conclusion The increased IL-31 and IL-33 levels plays an important role in the development of endometrial cancer, IL-31 combined with IL-33 has important diagnostic value of endometrial cancer.
