心脏植入性器械患者常规随访的时间和费用调查 被引量:1

The time and fee cost of conventional follow-up for cardiac implanted electrical device patients

摘要 目的评估常规随访对心脏植入性器械(CIED)患者生活影响,包括时间、费用和人力成本。方法本研究为多中心、前瞻性、观察性研究。CIED患者术后3个月到医院接受常规随访,并完善随访花费时间和费用的问卷调查。结果 2012年1月至2013年10月共入选161例患者,其中起搏器患者82例,埋藏式心脏转复除颤器患者39例,心脏再同步化治疗患者40例。患者年龄(62.3±14.29)岁,其中男性108例(67.08%)。患者平均花费150元。患者路途用时、挂号开单用时、检查前等待及检查时间分别为120、40、30及15 min,76.7%的患者需要陪同人员一同前往医院,67%的陪同人员需要请假。结论患者路途及检查前花费时间及由此产生住宿和交通的费用,大大增加了患者常规诊室随访的成本,远程监测随访可望大大节省患者的时间及金钱成本。 Objective To appraise the influence of conventional follow-up on the cardiac implanted electric devices (CIED) patients. Methods This was a multicenter, perspective and observational study, which enrolled patients who had been treated with CIED including pacemaker, implantable cardioverter defibrillators(ICD) and cardiac re- synchronization therapy(CRT). The patients were asked to do questionnaires about time and fee cost for the routine- ly follow-up 3 months after implantation. Results Totally 161 patiens from 12 medical centers, mean age of ( 62. 3±14.29)years old,67.08% male, were enrolled from January 2012 to October 2013. The average fee for follow-up was 150 yuan, mostly for the transportation and accommodation. The time-comsuming for transportation, register, periexamine waiting and CIED check-up was 120 min,40 min, 30 min and 15 min, respectively. 76.7% patients need family member company, 67% family member need ask for a leave. Conclusions The time consuming for transportation is a major component of the overall time-cost of conventional follow-up, so is the fee. Telemedi- cine-assisted long-term care may considerably reduce the time and fee cost.
