原发性高血压患者血压昼夜节律与靶器官损害的关系研究 被引量:21

Relationship between Circadian Rhythm of Blood Pressure and Target Organ Damage in Patients with Essential Hypertension

摘要 目的:研究原发性高血压患者血压昼夜节律异常是否与靶器官损害存在关联。方法:将2015年2~11月份在我院治疗的94例原发性高血压病人按照血压昼夜节律是否正常分为节律正常组52例和异常组42例。比较两组患者的24h、白天、夜间的收缩压(SBP)与舒张压(DBP)及血压负荷,并且比较两组患者心脏、脑、肾损伤相关指标。结果:异常组全天24h、白天、夜间SBP及DBP,血压负荷均高于正常组,差异具有统计学意义(P<0.05)。异常组患者左室重量(LVM)和左室重量指数(LVMI)、发生心肌缺血次数及持续时间、发生脑梗死几率均明显高于正常组,差异有统计学意义(P<0.05)。异常组尿微量白蛋白(MAU)、尿酸(UA)水平均高于正常组,差异有统计学意义(P<0.05)。结论:原发性高血压患者出现昼夜节律异常,可能对相关靶器官造成损伤。 Objective: To investigate relationship between the circadian rhythm of blood pressure and target organ damage in essential hypertension patients. Methods: 94 patients with essential hypertension in our hospital from February to November of 2015 were divided into normal group(52 cases) and abnormal group(42 cases) according to whether the circadian rhythm of blood pressure was normal. The systolic blood pressure(SBP),diastolic blood pressure(DBP) and blood pressure load were compared between the two groups at 24 h,day and night.Meanwhile,the heart and brain damage indexes were compared between the two groups. Results: The SBP and DBP at 24,day,night and blood pressure load in abnormal group were higher than normal group(P〈0.05). The levels of left ventricular mass(LVM) and left ventricular mass index(LVMI),frequency and duration of myocardial ischemia,incidence of cerebral infarction in abnormal group were higher than normal group(P〈0.05). The levels of microscale albuminuria(MAU) and uric acid(UA) in the abnormal group were higher than those in the normal group(P〈0.05). Conclusion: Circadian rhythm of blood pressure abnormality appeared in patients with essential hypertension may damage the related target organ.
