内脂素在妊娠期糖尿病患者中的检测价值

The detective value of serum visfatin in patients with gestational diabetes mellitus

目的探讨血清内脂素(visfatin,VF)在妊娠期糖尿病(gestational diabetes mellitus,GDM)中的临床意义及其与胰岛素抵抗(insulin resistance,IR)的关系。方法分析2015年5月至2016年5月在成都市妇女儿童中心医院接受产前检查且住院分娩的GDM孕妇(38例,观察组)和正常孕妇(38例,对照组)的临床资料,比较两组孕妇基础临床指标水平、糖尿病家族史、血清VF、空腹血糖(fasting plasma glucose,FPG)、空腹胰岛素(fasting insulin,FINS)及胰岛素抵抗指数(insulin resistance index,IRI)水平,分析两组孕妇血清VF与相关因素的关系。结果两组孕妇的孕周、身高、孕前体重及体质量指数(body mass index,BMI)、糖尿病家族史差异均无统计学意义(P>0.05);观察组孕妇诊断时的体重及BMI值、血清VF、FPG、FINS及IRI水平均明显高于对照组(P<0.05)。两组孕妇的血清VF水平与其孕前体重及BMI、诊断时体重及BMI、FPG均无相关性,但与FINS及IRI呈显著正相关(P<0.05)。结论血清VF在GDM孕妇的发病中发挥着重要的作用,其与IR有正相关性。

Objective To analyze the clinical value of serum visfatin（VF） in pregnant women with gestational diabetes mellitus （GDM） and the relationship between VF and insulin resistance. Methods Clinical data of 38 pregnant women with GDM （ observation group） and 38 normal pregnant women（ control group） received prenatal examination and hospital delivery in Chengdu Women and Children＇s Central Hospital from May 2015 to May 2016 was retrospectively analyzed. The levels of basic clinical indicators, family history of diabetes, serum levels of VF, fasting plasma glucose（ FPG）, fasting insulin（FINS） and insulin resistance index（IRI） of the two groups were compared. Relationship between serum VF and related factors of the two groups was analyzed. Results The gestational weeks, height, pre - gestational weight, body mass index （BMI） and family history of diabetes in the two groups had no statistical difference（P 〉 0. 05）. The weight and BMI at the time of diagnosis, serum levels of VF, FPG, FINS and IRI in the observation group were obviously higher than those in the control group （P 〈 0. 05）. Serum levels of VF had no correlation with pre - gestational weight and BMI, the weight and BMI at the time of diagnosis, as well as FPG in the two groups, but had positive correlationwith FINS and IRI（P 〈 O. 05）. Conclusion Serum VF plays an important role in the process of GDM. The level of serum VF has positive correlation with insulin resisitance.
