布地奈德治疗小儿肺炎支原体肺炎的效果及其对免疫球蛋白水平的影响 被引量:17

摘要 目的探讨布地奈德雾化吸入对肺炎支原体肺炎患儿的治疗效果及其对免疫球蛋白水平的影响。方法选取驻马店市某医院2013年1月-2015年1月收治的84例肺炎支原体肺炎患儿为研究对象,随机分为观察组与对照组,每组42例。对照组患儿静脉滴注阿奇霉素进行治疗,观察组患儿在对照组基础上加用布地奈德雾化吸入进行治疗,比较2组患儿治疗总有效率及治疗前后2组患儿的血清免疫球蛋白(IgG,IgM,IgA)水平。结果观察组患儿治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗前,2组患儿血清IgG,IgM,IgA水平差异无统计学意义(P>0.05);治疗后,2组患儿IgG,IgA水平低于治疗前,差异有统计学意义(P<0.05),且观察组患儿IgG,IgA水平均低于对照组,差异有统计学意义(P<0.05)。结论静脉滴注阿奇霉素联合布地奈德雾化吸入治疗小儿肺炎支原体肺炎效果显著,且能改善患儿血清免疫球蛋白水平,值得临床推广应用。 Objective To explore the efficacy of inhaled budesonide in treating mycoplasma pneumoniae (MP) pneumonia and its effect on immunoglobulin levels in children. Methods From January 2013 to January 2015, a total of 84 children with MP pneumonia were recruited and randomized into either the trial group (n:42) or the control group (n = 42). Children in the control group received intravenous azithromycin alone, while those in the trial group received intravenous azithromycin plus inhaled budesonide. The overall response rate and the serum levels of immunoglobulins (IgG, IgM and IgA) were compared between the two groups. Results The overall response rate was significantly higher in the trial group than that in the control group (P〈0.05). Before treatment, the serum levels of IgG, IgM and IgA were comparable between the two groups (P〉0.05). After treatment, the IgG and IgA levels in both groups were significantly lowered compared with those before treatment (P〈0.05) ; The IgG and IgA levels in the trial group were significantly lower than those in the control group (P〈0.05). Conclusion The addition of inhaled budesonide to intravenous azithromycin has favorable efficacy in treating children with MP pneumonia, which may also improve their immunoglobulin levels. These findings may have important implications for our clinical practice.
