血压、血脂、血糖及肾功能不同水平对老年高血压心血管事件的影响

Effects of levels of blood pressure,blood lipid,blood glucose and renal function on cardiovascular events in patients with senile hypertension

目的探讨血压、血脂、血糖及肾功能不同水平对老年高血压心血管事件的影响。方法选取老年高血压患者448例,以血压(收缩压SBP 140 mm Hg,舒张压DBP 80 mm Hg,脉压PP 60 mm Hg)、血脂(总胆固醇TC5.72 mmol/L)、血糖(空腹血糖FPG 7.0 mmol/L)及肾功能(估算肾小球滤过率e GFR 60 ml·min^(-1)·1.73-2)为界值分为低、高水平组。结果 2年随访期间统计冠心病事件52.90%(237/448),脑卒中事件24.11%(108/448),心血管死亡6.70%(30/448)。高SBP、DBP、PP、TC、FPG组患者冠心病事件、脑卒中事件与心血管死亡发生率明显高于低SBP、DBP、PP、TC、FPG组(P<0.05),低e GFR组上述指标明显高于高e GFR组(P<0.05)。结论血压、血脂、血糖及肾功能不同水平与老年高血压心血管事件的发生具有紧密关系,控制上述指标于正常范围在改善疾病预后状况中具有重要的意义。

Objective To investigate the effects of levels of blood pressure , blood lipid , blood glucose and renal function on cardiovascular events in patients with senile hypertension .Methods Four hundred and forty-eight patients with senile hypertension were enrolled in the study .These patients were divided into high level group and low level group by taking blood pressure （ SBP 140mm Hg, DBP 80mm Hg, PP 60mm Hg）, blood lipid （ TC5.72mmol/L）, blood glucose （ FPG 7.0mmol/L） and renal function （eGFR60ml· min·^ -2 · 1.73m^-2 ） as bound values.Results After 2-year follow up, the incidence rate of coronary heart disease （CHD） events was 52.90% （237/448）,stroke events 24.11% （208/448）, cardiovascular death 6.70% （30/448）.The incidence rates of CHD events ,stroke events and cardiovascular death in high level group were significantly higher than those in low level group ,moreover ,which in low eGFR group were significantly higher than those in high eGFR group （ P 〈0.05）.Conclusion The levels of blood pressure,blood lipid,blood glucose and renal function are closely correlated to cardiovascular events in patients with senile hypertension ,thus,it has an important meaning to control these indexes mentioned above in normal range for improving disease ＇ s prognosis .
