行气健脾法联合西药治疗肝硬化合并糖代谢异常疗效观察

The observation on curative effect of the treatment of liver cirrhosis with abnormal glucose metabolism by method of invigorating spleen and Qi combined with western medicine

目的观察行气健脾法联合西药治疗肝硬化合并糖代谢异常的临床效果。方法将92例肝硬化合并糖代谢异常患者随机分为观察组和对照组,每组46例。观察组采用行气健脾法,方选七味白术散加减,联合护肝、抗病毒、胰岛素及对症支持的西药治疗,对照组单用西药治疗,疗程均为3个月。观察治疗前后2组肝功能、血糖水平及肝硬化并发症发生情况。结果治疗后,2组空腹血糖(FBG)、餐后2 h血糖(2 hPG)较治疗前均下降(P<0.05或P<0.01),但2组治疗后数值比较差异无统计学意义(P>0.05)。2组治疗后肝功能各指标较治疗前均有所改善(P<0.05);且观察组改善程度优于对照组(P<0.05)。观察组并发症发生率为56.52%,低于对照组的76.19%(P<0.05)。结论行气健脾法联合西药治疗肝硬化合并糖代谢异常能够显著改善肝功能,减少并发症,但对控制血糖疗效不显著。

Objective To explore clinical curative effect of using method of invigorating spleen and Qi combined with western medicine in the treatment of liver cirrhosis with abnormal glucose metabolism. Methods 92 patients of liver cirrhosis with abnormal glucose metabolism enrolled in the randomized controlled clinical trial were divided into treatment group （46 cases） and control group （46 cases）. Patients in treatment group were treated with Qiwei Baizhu powder invigorating spleen and Qi, combined with Western medicine treatment about liver protection, antiviral, insulin and symptomatic support and those patients in control group only treated with western medicine, with 3 months treatment for both of the groups. The liver function, blood sugar level and liver cirrhosis complication were observed before and after treatment in the two groups. Results After treatment,two groups of FBG,2 hPG were significantly decreased compared with those of before treatment （P 〈 0.05 or P 〈 0. 01 ）. There were no differences between the two groups （P 〉 0.05 ）. All indicators included in this examination show that performance of liver function are improved after treatment （ P 〈 0.05 ）, and the level of improvement in treatment group are greater than that of in control group （P 〈 0.05 ）. The incidence of complication in treatment group is 56.52% lower than con- trol group of 76.19% （P 〈 0.05）. Conclusion Method of invigorating spleen and Qi combined with western medicine in the treatment of liver cirrhosis with abnormal glucose metabolism can significantly improve the liver function, reduce complications, but it has no effect on the control of blood sugar.
