经皮肾镜术中气压弹道碎石和Cyberwand超声双频导管碎石方式对血流动力学影响的对比研究 被引量:4

摘要 目的研究经皮肾镜术中气压弹道碎石和Cyberwand超声双频导管碎石方式对血流动力学的影响。方法选取我院2014年6月至2015年6月收治的60例肾结石患者,随机分为观察组和对照组各30例。观察组给予Cyberwand超声双频导管碎石方式治疗,对照组给予经皮肾镜术中气压弹道碎石治疗。观察并比较两组血流动力学。结果术前两组患者的SBP、DBP、SPO2比较差异均无统计学意义(P>0.05)。与术前比较,两组患者术中30 min、60 min的血压(收缩压/舒张压)均明显降低,但观察组术中90 min、120 min的血压逐渐趋于平稳,对照组仍有下降趋势,对照组对血压的影响比观察组更明显(P均<0.05)。两组术中血氧饱和度与术前比较差异均无统计学意义(P>0.05)。结论 Cyberwand超声双频导管碎石方式具有可行性,对血流动力学影响较小,可临床推广。 Objective To study the influence of percutaneous pneumatic lithotripsy and Cyberwand ultrasonic dnal-frequency catheter lithotripsy on hemodynamics. Methods 60 cases of patients with kidney stone admitted to our hospital from June 2014 to June 2015 were selected and randomly divided into observation group and control group, with 30 cases in each group. The observation group was given Cyherwand ultrasonic dual-frequency catheter lithotripsy, while the control group was given percutaneous pneumatic lithotripsy. The hemodynamics was observed and compared between two groups. Results No statistical difference was found in the SBP, DBP and SPO2 between two groups before surgery (P 〉0.05). Compared with that before surgery, the blood pressure (systolic pressure and diastolic pressure) of two groups significantly decreased at 30 min and 60 min during surgery, but the blood pressure of observation group at 90 min and 120 min during surgery tended to be stable while that of control group remained decreasing; Influence on blood pressure in control group was more obvious than that of observation group (P 〈0.05). After surgery, the oxyhemoglobin saturation of both groups had no statistical difference compared with that before surgery (P 〉0.05). Conclusions Cyberwand ultrasonic dual-frequency catheter lithotripsy is feasible with less influence on the hemodynamics, which is worthy of clinical promotion.
