超声弹性成像应变率比值法与改良5分法鉴别诊断乳腺病灶良恶性的临床价值 被引量:4

摘要 目的探讨超声弹性成像应变率比值法与改良5分法鉴别诊断乳腺病灶良恶性的临床价值,为临床诊断和治疗提供理论依据。方法选取我院2013年5月至2015年6月收治的乳腺病变患者作为研究对象,针对经手术病理证实的220个乳腺病灶进行超声弹性成像检查,分别应用应变率比值法和改良5分法进行评价,以病理结果作为金标准比较两种方法在鉴别诊断病灶良恶性方面的临床价值。结果超声检查结果显示,良性病灶147个,恶性病灶73个;恶性乳腺病灶S R值明显高于良性病灶,差异有统计学意义(P<0.05);超声弹性成像应变率比值法的灵敏度、特异度和准确度与改良5分法比较,差异无统计学意义(P>0.05)。结论两种方法均能够鉴别诊断乳腺疾病的良恶性,但应变率比值法具有更强的客观性,值得临床应用。 Objective To explore the clinical value of ultrasound elastography strain ratio method and ~mproved 5-point metiaod m differential diagnosis of benign and malignant breast lesions, in order to provide basis for clinical diagnosis and treatment. Methods Patients with breast lesions admitted to our hospital from May 2013 to June 2015 were selected as the research objects. 220 breast lesions confirmed by surgery and pathology were given ultrasonic elastography, and strain rate ratio method and improved 5-point method were used to evaluate. As the gold standard of pathology results, the clinical values between two kinds of methods in differential diagnosis of benign and malignant lesions were compared. Results The results of ultrasonography showed 147 benign lesions and 73 malignant lesions; The SR value of malignant breast lesions was significantly higher than that of benign lesions, with statistical difference (P 〈0.05). No statistical difference was found in the sensitivity, specificity and accuracy between ultrasound elastography strain ratio method and improved 5-point method (P 〉0.05). Conclusions Both two methods can differentiate and diagnose benign and malignant breast diseases, but strain ratio method is more objective, which is worthy of clinical application.
