局部麻醉和脊椎麻醉在膝关节镜手术中的效果及安全性比较

Comparison on the Effect and Security between Local Anesthesia and Spinal Anesthesia in Knee Arthroscopy

目的分析比较局部麻醉和脊椎麻醉在膝关节镜手术中的效果及安全性。方法选取我院2015年6月至2016年7月收治的88例接受膝关节镜手术治疗的患者,随机分为对照组与观察组,各44例。对照组给予脊椎麻醉,观察组给予局部麻醉。结果两组的麻醉效果满意度和术中VAS评分比较差异均无统计学意义(P>0.05);观察组的术后VAS评分显著低于对照组(P<0.05);观察组术后排尿功能恢复时间为(91.56±18.71)min,显著短于对照组的(176.12±23.86)min(P<0.05);观察组的并发症发生率为2.27%,明显低于对照组的15.91%(P<0.05)。结论局部麻醉应用于膝关节镜手术中具有操作简单、麻醉效果好、术后并发症少的优点,安全可靠,值得临床推广应用。

Objective To compare and analyze the effect and safety between local anesthesia and spinal anesthesia in knee arthroscopy. Methods 88 cases of patients undergoing knee arthroscopy in our hospital from June 2015 to July 2016 were selected and randomly divided into control group and observation group, with 44 cases in each group. The control group was given spinal anesthesia, while the observation group was given local anesthesia. Results No statistical difference was found in the satisfaction degree of anesthesia and intraoperative VAS score between two groups （P〉0.05）. The postoperative VAS score of observation group was significantly lower than that of control group （P 〈0.05）. The recovery time of urinary function after operation of observation group was （91.56 ± 18.71） min, significantly shorter than （176.12 ± 23.86） min of control group （P〈0.05）. The incidence of complications of observation group was 2.27%, significantly lower than 15.91% of control group （P 〈0.05）. Conclusions Local anesthesia in the application of knee arthroscopy has the advantages of simple operation, good anesthetic effect and fewer postoperative complications, with safety and reliability, which is worthy of clinical promotion and application.
