巨大眼内异物并眼内炎玻璃体切除联合手术的效果

The efficacy of vitrectomy combined surgery for the treatment of huge intraocular foreign body and coexisted endophthalmitis

目的观察巨大眼内异物并眼内炎施行玻璃体切除联合异物摘出及硅油填充术的效果。方法2010年1月至2013年11月对10例（10眼）长度10—25mm的巨大眼内异物并眼内炎的资料进行回顾性分析。所有患者治疗前均常规抽取玻璃体积脓送细菌培养及真菌培养，并做药物敏感试验。进行玻璃体切除联合异物摘出及硅油填充手术，术后随访6—12个月，观察分析术后视力和眼压改善情况。结果10眼均一次手术成功摘出眼内异物，眼内炎得到控制；8眼视力不同程度提高。结论玻璃体切除术联合眼内异物摘出及硅油填充术能安全摘出巨大眼内异物并有效控制眼内炎和提高视力。

Objective To observe the clinical efficacy of vitrectomy combined with extraction of intraocular foreign body and silicone oil tamponade for the treatment of huge intraocular foreign body and coexisted endophthalmitis. Methods The clinical data of 10 eyes of 10 patients with the length of 10-25 mm huge intraocular foreign body and coexisted endophthalmitis were analyzed retrospectively. Vitreous purulence was taken before surgery for bacterial and fungal cuhure and drug sensitivity test. And then all cases underwent vitrectomy combined with extraction of intraocular foreign body and silicone oil tamponade . The follow-up time was 6 to 12 months. The postoperative improvement in visual acuity and intraocular pressure were observed. Results Intraocular foreign bodies of 10 cases were extracted successfully in one operation. All cases of endophthalmitis were controlled. The corrected visual acuity improved in 8 cases. Conclusion Vitrectomy combined with extraction of intraocular foreign body and silicone oil tamponade is favorable and safe for the treatment of huge intraocular foreign body and coexisted endophthalmitis.
