摘要
目的观察爆竹所致眼外伤患者滑车上动脉的彩色多普勒的超声改变特征。方法前瞻性观察我院2016年2月8日至2016年2月14日收治的爆竹所致眼外伤28例(28眼)。所有患者术前均行彩超检查双侧眼球结构及滑车上动脉。以健康人20例(40眼)做为对照组,观察并比较两组的滑车上动脉收缩期血流速度峰值(Vs)、舒张末期血流速度(Va)及阻力指数(RI)。结果两组患者的滑车上动脉检出率为100%,其频谱特征与眼动脉相似,为正向三峰型:SP1峰、SP2峰及DP峰.SP1〉SP2。外伤后滑车上动脉Vs明显增高,Vd明显降低,RI增高,差异有统计学意义(P〈0.01),且外伤情况越严重,RI增高程度越高。结论外伤后滑车上动脉频谱收缩期高尖,舒张期低平,呈高速高阻状态。该提示滑车上动脉有痉挛,或者末梢血管有梗阻,可证明眼部血供受到影响,且RI敲高俞后拙善.
Objective To observe the characteristics of the supratrochlear artery (STCA) hemodynamics in ocular injuries caused by firecracker with colour Doppler ultrasound. Methods The prospective observational study included and 28 eyes of 28 cases of ocular injury caused by firecracker from 8th Feb. 2016 to 14th Feb. 2016. All patients of bilateral eyeball structure and STCA were detected with colour Doppler ultrasound. Forty eyes of 20 normal persons ( control group) were compared with the injured group. The systolic peak velocity of STCA , the diastolic velocity of STCA and the resistance index were observed and compared between the two groups. Results The detection rate of STCA in two groups was 100%. The characteristics in fi'equency spectrum of STCA , similar to ophthalmic artery , was a triple positive peak type: SP1, SP2 and DP. SP1 was more than SP2. After injured the Vs and RI of STCA increased significantly and the Vd of STCA decreased obviously. The difference in three indexes between the two groups were statistically significant ( P 〈 0.01 ). The extent of eyeball injured was positively correlated with the rise of RI of STCA. Conclusion After injured STCA has a higher systolic peak velocity, a lower the diastolic velocity and a higher vascular resistance index. It means STCA with spasticity. The extent of eyeball injured is positively correlated with the rise of RI of STCA.
