摘要
目的探讨聚维酮碘稀释液结膜囊冲洗联合左氧氟沙星滴眼预防白内障术后感染疗效,并观察和分析其预后,为临床预防和降低白内障术后感染提供依据。方法抽选2015年1月—2016年12月我院接收白内障患者200例(318眼),随机分为对照组100例(156眼)和治疗组100例(162眼)。两组患者行白内障超声乳术、植入人工晶体。对照组患者治疗方案:术前3天,开始用0.5%左氧氟沙星眼药水,每日滴眼4次,持续滴眼至手术前20分钟。治疗组患者治疗方案:在对照组的基础上,应用浓度为0.05%的聚维酮碘对结膜囊进行有效冲洗。评估两组患者冲洗前、后及术后第2天细菌培养结果及术后不良反应。结果两组在冲洗后及术后第2天阳性率均低于冲洗前,差异具有统计学意义(P<0.05)。术后及术后第2天,治疗组中各有1例(0.62%)感染,对照组有9例(5.77%)和10例(6.41%)感染。两组数据相比,差异具有统计学意义(P<0.05)。结论聚维酮碘稀释液结膜囊冲洗联合左氧氟沙星滴眼预防白内障术后感染具有较好的临床疗效,能够减少细菌培养阳性率,降低患者不良反应发生率。
Objective To investigate the efficacy of povidone iodine diluent conjunctival sac irrigation combined with levofloxacin eye drops in prevention of infection after cataract surgery, the prognosis was observed and analyzed, to provide the basis for clinical prevention and reduction of postoperative infection. Methods 200 cases of cataract patients(318 eyes) from January 2015 to December 2016 in our hospital were selected as the study object and were randomly divided into 100 cases of patients in control group(156 eyes) and 100 cases of patients in treatment group(162 eyes). Two groups of patients underwent phacoemulsification and intraocular lens implantation. Patients in control group: 3 days before surgery, began to use levofloxacin eye drops, eye drops 4 times once a day, continued to drop to the eye before surgery for 20 minutes. Patients in the treatment group: on the basis of the control group, the concentration of 0.05% povidone iodine was used to rinse the conjunctival sac. The result of two groups of patients before and after washing, and the results of bacterial culture and postoperative adverse reactions were observed on the second day after operation. Results The positive rate of the two groups was lower than that of before and after washing in the second groups, the difference was statistically significant(P〈0.05). There were 1 cases(0.62%) in thetreatment group after operation and on the second day after operation, In the control group, 9 cases(5.77%) and 10 patients(6.41%) were infected. The difference between two groups was statistically significant(P〈0.05). Conclusion To observe the clinical efficacy of povidone iodine diluent conjunctival sac irrigation combined with levofloxacin eye drops in prevention of postoperative infection, which can reduce the positive rate of bacterial culture, to reduce the incidence of adverse reactions.
