摘要
目的探讨右美托咪定预先给药在实施胃癌根治术的高血压患者中的应用效果。方法选取2015年5月~2016年5月在我院实施腹腔镜胃癌根治术的高血压患者108例为研究对象,将其按照随机数字表分为对照组和观察组,每组54例。麻醉诱导前给予观察组患者右美托咪定(0.6μg/kg),对照组给予等量生理盐水。于麻醉诱导前(T0)、拔管时(T1)、拔管后5 min(T2)、拔管后10 min(T3),观察两组患者的HR、MAP及Sp O2指标,并比较两组的不良反应发生率。结果两组患者在T0时段的各项指标差异无统计学意义(P>0.05),且T1~T3时段的Sp O2差异无统计学意义(P>0.05);在T1~T3时段,观察组患者的心率与平均动脉压较对照组低(P<0.05);对照组与观察组患者的不良反应发生率分别为38.89%、14.81%,差异有统计学意义(P<0.05)。结论右美托咪定预先给药应用于实施腹腔镜胃癌根治术的高血压患者,可减少其全身麻醉苏醒期的躁动和不良反应。
Objective To explore the application effect of premedication of Dexmedetomidine in hypertensive patients undergoing radical gastrectomy.Methods From May 2015 to May 2016,108 patients with hypertension treated by laparoscopic radical gastrectomy in our hospital were selected as study objects and they were randomly divided into control group and observation group,with 54 cases in each group.Patients in the observation group were given Dexmedetomidine(0.6 μg/kg) before the ind uction of anesthesia,and in the control group,the same volume of saline was provided.Heart rate(HR),mean arterial pressure(MAP) and oxyhemoglobin saturation(Sp O2) were observed before anesthesia induction(T0),extubation(T1),5 minutes after extubation(T2),and 10 minitues after extubation(T3).The incidence rate of adverse reactions between two groups was compared.Results There was no significant difference between the two groups in T0 period and the difference of Sp O2 from T1-T3 was not significant(P〈0.05).At T1-T3,indexes of HR and MAP in the observation group were both lower than those in the control group(P〈0.05).The incidence rate of adverse reactions in the control group and observation group was 38.89% and 14.81% with a statistical difference(P〈0.05).Conclusion Premedication of Dexmedetomidine can reduce the restlessness and adverse reaction of general anesthesia during recovery period.
