摘要
目的探讨可溶性细胞黏附因子-1(sICAM-1)、s-100β蛋白及高迁移率族蛋白1(high mobility group box-1protein,HMGB1)在动脉瘤性蛛网膜下腔出血后脑积水的诊断价值。方法分析2014年1月~2016年7月在我院接受显微外科动脉瘤夹闭术的87例动脉瘤性蛛网膜下腔出血患者的临床资料,45例术后未出现脑积水患者纳入对照组。结果观察组患者第1、7、14天内血清sICAM-1、s-100β及HMGB1水平明显高于对照组,差异均具有统计学意义(P<0.05);观察组IV^V级组患者血清sICAM-1、s-100β及HMGB1水平明显高于I^III级组患者,差异具有统计学意义(P<0.05);由ROC曲线分析显示,当s-100β取值为1.74μg/l时对动脉瘤性蛛网膜下腔出血伴脑积水的诊断敏感性为92.86%,特异性为82.27%,相比sICAM-1、HMGB1,诊断价值较佳。结论 aSAH后脑积水发生可能与血清sICAM-1、s-100β及HMGB1水平异常升高有关,其中s-100β可用于aSAH后脑积水发生的早期监测,具有诊断价值。
Objective To investigate the value of slCAM-1, s-100βand HMGB1 in the diagnosis of hydrocephalus after aneurysmal subarachnoid hemorrhage. Methods The clinical data of 87 cases of patients with subarachnoid hemorrhage in our hospital from January 2014 to July 2016 were analyzed, and 42 patients with hydrocephalus after surgery were selected as observation group. 45 patients without hydrocephalus after surgery were selected as control group. Results The serum levels of sICAM-1 ,s-100β and HMGB1 in the observation group were significantly higher than those in the control group at 1,7,14 days, and the difference was statistically signifi- cant (P〈0.05) ;The level of serum sICAM-1, s-100β, and H MGB1 in IV-V of the observa- tion group were significantly higher than those in the I-III group, the difference was statistically significant (P〈0.05) ;By the analysis of ROC curve,it showed that when s-100β value was 1.74 μg/l,the diagnostic sensitivity of aneurysmal subarachnoid hemorrhage with hydrocephalus was 92.86% ,the specificity was 82.27% ,compared with sICAM- 1, HMGB1, the diagnostic value of s-100β was best. Conclusion sashwith Hydrocephalus may be related to elevate of serum levels of slCAM-1,s-100β and HMGB1, which can be used for early monitoring of ash with hydro- cephalus,and it have the diagnostic value.
