五倍子汤熏洗结合四黄膏外敷治疗痔术后疼痛39例临床观察

Observation of Wubeizi Decoction fumigation combined with Sihuang Gao in the treatment of the pain after hemorrhoid surgery

目的:观察五倍子汤熏洗结合四黄膏外敷对痔术后疼痛的临床疗效。方法:选取78例痔术后疼痛住院患者,按随机数字表法随机分为治疗组和对照组各39例。两组术后均予常规治疗,在常规治疗的基础上,对照组予1∶5000高锰酸钾溶液熏洗,治疗组予五倍子汤熏洗结合四黄膏外敷,观察并记录两组肛门疼痛视觉模拟评分(VAS)及并发症状。结果:两组术后第1天,VAS评分差异无统计学意义(P>0.05);术后第3天及第7天,VAS评分差异均有统计学意义(P<0.05)。对照组有2例出现术后轻度肛门水肿症状,经持续观察后症状减轻,有2例出现术后出血症状,经持续观察和换药后症状消失,有1例疼痛剧烈,口服止痛药后症状缓解。治疗组有1例出现术后出血症状,经持续观察和换药后症状消除。结论:五倍子汤熏洗结合四黄膏外敷能有效减轻痔术后患者的疼痛症状,并减少并发症的发生,是治疗痔术后疼痛的安全有效方式,值得推广应用。

Objective To observe the effects of Wubeizi Decoction fumigation combined with Sihuang Gao treat the pain after hem- orrhoid surgery. Methods Screening of 78 cases of postoperative pain after hemorrhoids surgery, divided into treatment group and con- trol group at random according to the number of hospitalization, routine treatment was given to the two groups of patients after opera- tion, on the basis of conventional therapy, the control group was given 1 ： 5000 Potassium Permanganate solution fumigation, treatment group was treated with Wubeizi Decoction fumigation combined with Sihuang Gao. Visual Analogue Scale （VAS） and concurrent symptoms of two groups of patients were observed and recorded. Results There was no significant difference in VAS scores between the two groups the first days after operation （ P 〉 0. 05 ）. VAS score differences were statistically significant between the two groups the third days and the seventh days after operation （P 〈 0. 05）. In the treatment group, 1 cases of postoperative bleeding symptoms were observed and the symptoms were eliminated after dressing change ; in the control group, 2 cases of patients with mild postoperative anal edema symptom, the symptom relief after continuous observation, there were 2 cases of postoperative bleeding symptom, the symptom disappeared after continuous observation and dressing change, there was 1 patient with severe pain, and the symptom was relieved after oral administration of analgesics. Conclusion Wubeizi Decoction fumigation combined with Sihuang Gao can effectively reduce postop- erative pain in patients after hemorrhoid surgery, and reduce the incidence of complications, it is a safe and effective way to treat the postoperative pain of hemorrhoids.
