上颌后牙区骨量严重不足时采用改良式IOSFE技术的临床研究 被引量:7

摘要 目的研究当上颌后牙区剩余牙槽嵴高度(RBH)不足4 mm时,采用改良式上颌窦底内嵌骨块提升技术(IOSFE)同期植入种植体的骨增量效果及临床疗效。方法纳入27例上颌后牙区RBH不足患者,行改良式IOSFE同期植入种植体共37枚,术前术区牙槽嵴高度为0.56~3.99(2.24±1.05)mm。术后当天及术后6个月拍摄口腔颌面锥形束CT(CBCT),观察种植体周围骨结合情况;借助三维重叠技术,测量分析术区骨高度变化。结果随访期内种植体存留率100%,无相关并发症发生。种植体稳定性良好,上部修复体可正常行使咀嚼功能。术后CBCT及三维重叠图像显示,术后当天上颌窦底至牙槽嵴顶高度较术前明显增加,平均值为(11.09±1.58)mm;术后6个月RBH平均为(9.13±1.87)mm,较术前平均增加(6.89±1.99)mm,较术后当天平均减少(1.95±1.05)mm。结论当上颌后牙区RBH<4 mm时,改良式IOSFE技术同期植入种植体操作简单快捷、成功率较高,且骨增量效果显著,骨结合良好,是一种可获得较好临床疗效的改良式IOSFE技术。
