两种缝线固定方式在翼状胬肉切除术后舒适度的对比研究

Comparison of comfort between two methods of sutures after pterygium surgery

目的探讨两种缝合方式对翼状胬肉术后患者舒适度的影响。方法临床病例对照研究。回顾性分析2012年1月至2014年1月在解放军第105医院行双眼翼状胬肉切除手术的患者共40例80只眼，其中男17例34只眼，女23例46只眼，随机1只眼予以10-0缝线间断缝合固定结膜瓣（A组）；另1只眼予以10-0缝线连续加锁缝合固定结膜瓣（B组），对术后患者眼部畏光流泪、异物感、眼痛、球结膜充血等舒适度方面进行评分。结果 翼状胬肉切除联合角膜缘干细胞移植术中，术后1、3、7d及1月A组舒适度评分分别为（14．25±0．16）分、（9．63±0．23）分、（5．65±0．11）分和（1．25±0．11）分，B组舒适度评分分别为（13．53±0．15）分、（8．70±0．20）分、（5．33±0．08）分和（0．93±0．08）分，两组差异有统计学意义（P〈0．05）；术后3月A、B组舒适度评分分别为（0．10±0．06）分、（0．05±0．03）分，差异无统计学意义（P〉0．05）。结论翼状胬肉切除联合角膜缘干细胞移植术应用连续加缝合方法术后舒适度优于间断缝合，值得推广。

Objective To investigate the effect of interrupted suture and continuous interlocking suture for eye comfort after pterygium surgery. Methods A retrospective series study. Forty patients with bilateral primary pterygium （80 eyes）, who were hospitalized in the 105th hospital of PLA from January 2012 to January 2014 including 17 male （34 eyes） and 23 female （46 eyes）, randomly chosen one eye in group A received interrupted suture using the 10-0 non-absorbable, the other eye in group B received continuous interlocking suture using the 10-0 non-absorbable, eye comfort was observed at 1-day, 3-day,7-day, 1-month and 3-month after the surgery. Results The 1-day, 3-day, 7-day and a month after the surgery, the comfort scores of group A were 14.25±0.16, 9.63±0.23, 5.65±0.11 and 1.25±0.11, the comfort scores of group B were 13.53±0.15, 8.70±0.20, 5.33±0.08 and 0.93±0.08, there was significant difference between the two groups （P 〈0.05）; But three-month after the surgery, the comfort score of group A and group B were （0.10±0.06） and （0.05±0.03）, there was no significant difference between the two groups （P 〉0.05）. Conclusions Continuous interlocking su- ture is better than interrupted suture in the surgery of pterygium excision combined with autologous corneal limbus stem cell transplantation, is worth promoting.
