普罗布考对脑梗死进展期患者APTT、FIB与血液流变学的影响 被引量:9

摘要 目的:考察普罗布考对脑梗塞进展期患者活化部分凝血活酶时间(APTT)、血浆纤维蛋白原(FIB)与血液流变学的影响。方法:选取本院82例脑梗塞进展期患者随机分为实验组和对照组,其中实验组42例予以普罗布考,0.25 g/次,每天2次;对照组40例按常规方法治疗。持续治疗两周后,对比分析两组患者在治疗前后APTT、FIB与血液流变学的改善情况。结果:(1)实验组和对照组患者收缩压、舒张压、心率、甘油三酯和总胆固醇水平,空腹血糖和谷丙转氨酶水平组间比较无明显差异,P>0.05。但空腹血糖、甘油三酯和总胆固醇水平略偏高;(2)组间比较实验组参数值APTT明显高于对照组,FIB明显低于对照组,其中实验组APTT增加(6.6±0.24)s,对照组增加(3.7±0.17)s,实验组FIB减少(2.36±0.11)g/L,对照组减少(1.14±0.10)g/L,差异显著,P<0.05;(3)治疗后两组患者血液流变学(包括高、低切变率、血浆黏度、红细胞压积)均有所改善,且实验组血液切变率、血浆粘度较对照组明显降低,实验组高切变率降低(1.6±0.6),低切变率降低各(4.7±1.2),血浆黏度降低(0.5±0.07),而对照组参数值改变分别为(1.1±0.9)、(3.6±1.0)、(0.1±0.09),二者差异显著,P<0.05,差异有统计学意义。结论:普罗布考能够显著改善进展期脑梗塞患者APTT、FIB与血液流变学参数,疗效显著,值得临床推广应用。 Objective:To study the effect of probucol on the level of on activated partial thromboplastin time (APTT) , plasma fibrinogen (FIB) and the blood rheology at the stage of advanced cerebral infarction. Methods:82 patients undergoing the advanced cerebral infarction were randomly divided into experimental group (42 cases) and control group( 40 cases). The experimental group were given probucol,0.25 g/times,2 times a day;While the control group were treated by routine method. After treatment, APTT, FIB and the blood rheology were compared before and after treatment. Results: ( 1 ) Systolic blood pressure, diastolic blood pressure, heart rate, fasting blood glucose, triglycerides and total cholesterol levels have no significant difference between the experimental group and the control group, but fasting glucose, triglycerides and total cholesterol levels are slightly higher. (2)APTT and FIB were improved in both groups after treatment, APTT in the experimental group increased (6.6 ±0.24)s, which was higher than (3.7 ±0.17 )s in the control group ; FIB in the experimental group reduced (2.36 ±0.11 ) g/L, which was lower than ( 1.14 ± 0.10) g/L in the control group with statistically significant difference (P 〈 0.05 ) ; ( 3 ) The blood rheology were improved in two groups after treatment ( including high shear rate,low shear rate,plasma viscosity). Experimental high shear rate decreased 1.6 ±0.6,low shear rate decreased 4.7 ±1.2, plasma viscosity decreased 0.5 ±0.07, while the control group parameter value changes were 1.1 ± 0.9,3.6 ±1.0,0.1 ± 0.09, respectively. The differences between two groups were statistically significant ( P 〈 0. 05 ). Conclusions: Probucol could firmly improve the APTT, FIB and hemorheology of cerebral infarction at the advanced stage,which shows a worthy of clinical application.
