摘要
目的 :用胸腔注射尿激酶疗法和抗结核疗法对结核性胸膜炎患者进行治疗的效果。方法 :将某院收治的66例结核性胸膜炎患者作为本次研究的对象。将这些患者随机平均分为观察组和对照组。在这些患者入院后,为其均使用3HRZE/9HR方案进行抗结核治疗。在此基础上,经胸腔静脉导管为观察组患者注射尿激酶。治疗结束后,比较两组患者治疗的效果。结果:进行治疗后,观察组患者进行治疗的总有效率明显高于对照组患者,差异有统计学意义(P<0.05)。观察组患者的PAI-1/t-PA值高于对照组患者,其胸膜的厚度优于对照组患者,差异有统计学意义(P<0.05)。结论:用胸腔注射尿激酶疗法和抗结核疗法对结核性胸膜炎患者进行治疗的效果较为理想。
Objective To observe the clinical effect of intrapleural urokinase combined with anti-tuberculosis regimens in the treatment of tuberculous pleurisy. Methods66 patients with tuberculous pleurisy treated in our hospital from May 2013 to December 2014 were selected as the research objects, they were divided by random number table into the observation group and the control group （each 33 cases）, two groups received chest tube drainage, control group received 3HRZE/9HR anti-tuberculosis regimen treatment, observation group added injection urokinase on the basis of the above, effective rate in the two groups were compared, pleural effusion plasminogen activator inhibitory factor-1/tectotype plasminogen activator ratio （PAI-1/t-PA）, changes of pleural thickness before and after the treatment were recorded, adverse reactions were observed. ResultsEffective rate in observation group 96.97% was significantly higher than control group （78.79%） （P〈0.05）; PAI-1/t-PA value （0.24 ± 0.04） in the observation group after the treatment was significantly higher than control group （P〈0.05）, pleural thickness （1.45 ±0.18）mm in observation group was significantly lower than control group （P〈 0.05）; there was no significant difference in incidence of adverse reactions in the two groups （9.09%, 6.06%） （P〉 0.05）. ConclusionIntrapleural urokinase combined with anti-tuberculosis regimens can increase PAI-1/t-PA ratio effectively, reduce pleural thickness with slight side effects, is worth for application in clinical experience.
