摘要
目的研究食管鳞状细胞癌(esophageal squamous cell carcinoma,ESCC)组织中叉头框蛋白C2(forkheadbox C2,FOXC2)、上皮-间质转化(epithelial-mesenchymal transition,EMT)相关标记物和血管生成拟态(vasculogenic mimicry,VM)形成相关标志分子的表达、相互之间的关系及与临床病理参数的关系。方法收集120例ESCC组织,采用免疫组织化学法检测ESCC中FOXC2、VM标志性分子血管内皮钙粘素(vascular endothelial cadherin,VE-cadherin)和血管内皮生长因子(vascular endothelial grow factor,VEGF)以及EMT相关标记物E-钙粘附蛋白(endothelial cadherin,E-cadherin)和波形蛋白(virmentin)表达;利用CD34/PAS套染观察ESCC中VM形成;分析FOXC2与ESCC临床病理特征的关系以及各指标之间的相互关系。结果在120例ESCC中,25例存在VM;FOXC2阳性表达率在浸润至深肌层及外膜层组、淋巴结转移组、Ⅲ+Ⅳ期组、低分化组、脉管癌栓形成组明显增高;FOXC2、vimentin在有VM病例中阳性表达率明显高于无VM病例,且均与VM形成呈正相关;E-cadherin在有VM病例中阳性表达率明显低于无VM病例,并与VM形成呈负相关;VEcadherin、VEGF和vimentin在FOXC2阳性病例中的FOXC2阳性表达率明显高于FOXC2阴性病例,且均与FOXC2表达呈正相关;E-cadherin在FOXC2阳性病例中的阳性表达率明显低于FOXC2阴性病例,并与FOXC2表达呈负相关。结论FOXC2可能通过EMT途径参与食管鳞癌中VM形成过程从而促进肿瘤的浸润转移;FOXC2的高表达提示其可以作为ESCC患者不良预后的一种新的独立指标。
Objective To study the expression of forkhead box C2 ( FOXC2), epithelial-mesenchymal transition (EMT) -related markers and vaseulogenic mimicry (VM)-related markers, and their relationships each other as well as with the clinical pathological characteristics in esophageal squamous cell carcinoma (ESCC). Methods 120 specimens of ESCC with clinical data were collected, in which the expression of FOXC2, VM-related markers vascular endothelial cadherin (VE-cadherin) and vascular endothelial grow factor ( VEGF), as well as EMT-related markers endothelial cadherin (E-cadherin) and vimentin was detected by immunohistoehemistry. Du- al staining by CD34 and periodic aeid-Sehiff (PAS) was performed to observe the formation of VM in ESCC. The relationship between FOXC2 and the clinical pathological characteristics of ESCC, and between the markers examined were analyzed. Results 25 cases of VM were found in the 120 samples. The positive rate of FOXC2 was much higher in the groups of stages III and IV tumors, poorly dif- ferentiated with cancer emboli, with infiltration into deep muscular and adventitial layers, as well as with lymph node metastasis. The positive rates of FOXC2 and vimentin were obviously higher in cases with than without VM, both were positively correlated with VM. The positive rate of E-cadherin was obviously lower in cases with than without VM, and its expression was negatively correlated with VM. The positive rates of VE-cadherin, vimentin and VEGF were obviously higher in FOXC2-positive cases than in FOXC2-negative cases, the expression of them being positively correlated with FOXC2. The positive rate of E-cadherin was obviously lower in FOXC2-positive cases than in FOXC2-negative cases, and its expression was negatively correlated with FOXC2. Conclusion FOXC2 might be involved in the formation of VM via EMT and promote tumor infiltration and metastasis; its high expression can be considered a new in- dependent indicator of poor prognoses of ESCC.
