摘要
目的研究肝细胞癌(hepatocellular carcinoma,HCC)患者癌组织中survivin、PTEN、EGFR表达水平及临床意义。方法选择2013年2月—2015年2月于白求恩国际和平医院行外科手术切除的HCC患者50例(手术病理组织实验证实)为观察组,同时取其中35例被病理组织实验证实无癌细胞浸润的癌旁组织作为对照组。均采用免疫组化染色检测观察组和对照组中的survivin、PTEN、EGFR表达水平。结果 survivin在观察组和对照组中阳性率分为72.0%、5.7%;PTEN在观察组和对照组中阳性率分别为52.0%、94.2%;EGFR在观察组和对照组中阳性率分别为74.0%、17.1%;差异均有统计学意义。survivin阳性情况与TNM分期存在联系(P<0.05);EGFR阳性情况和手术后2年内复发情况、临床分期和病理分期存在联系(P<0.05);PTEN阳性情况和手术前病灶转移、手术后2年内复发情况和TNM分期存在联系(P<0.05)。COX多因素分析中检测结果表示TNM分期、EGFR和survivin表达是HCC的预后因子。结论 survivin、PTEN和EGFR在HCC患者人体内呈现为高表达,EGFR和survivin可作为HCC的预后因子。
Objective To study the expressions of survivin,PTEN and EGFR in hepatocellular carcinoma(HCC) and their clinical significance.Methods Fifty patients(confirmed by surgical pathology) who underwent surgical resection between Febr uar y 2013 and Febr uar y 2015 were selected as the observation group.Thirty-f ive of them were conf irmed as having non-cancerous tissues by histopathological examination and ser ved as the control group.The expressions of survivin,PTEN and EGFR in bothl groups were detected by immunohistochemical staining.Results The positive rates of Survivin in the observation group and the control group were 72.0% and 5.7% respectively while the positive rates of PITE were 52.0% and 94.2% respectively.The positive rates of EGFR in the observation group and control group were 74.0% and 17.1% respectively.The positive expression of survivin was closely related to the TNM stage(P〈0.05).The positive rate of EGFR was correlated with the recurrence,clinical stage and pathological stage in 2 years after operation(P〈0.05).PTEN positive conditions were related to preoperative lesion metastasis.There was a close correlation between recurrence and TNM stage(P〈0.05).The COX multivariate analysis showed that TNM stage,EGFR and survivin expression were prognostic factors of HCC.Conclusion Survivin,PTEN and EGFR are highly expressed in patients with HCC.EGFR and survivin can serve as prognostic factors for HCC.
