水凝胶敷料联合复方七叶皂苷钠凝胶用于机械性静脉炎护理中的疗效观察

Observation of the curative effect of hydrogel dressing combined with compound sodium aescinate gel in the nursing of mechanical phlebitis

目的观察水凝胶敷料联合复方七叶皂苷钠凝胶治疗PICC在机械性静脉炎护理中的疗效。方法收集我科2013年1月~2016年6月共64例PICC带管期发生机械性静脉炎的患者,分为两组,观察组34例,对照组30例。2015年1月~2016年6月观察组采用水凝胶敷料联合复方七叶皂苷钠凝胶治疗,2013年1月~2014年12月对照组采用50%硫酸镁湿敷治疗,观察两组治疗机械性静脉炎的疗效、红肿消退时间、疼痛减轻时间、舒适度分值、护理满意度情况。结果观察组7 d内治愈18例,显效9例,有效6例,无效1例。对照组7d内治愈4例,显效5例,有效13例,无效8例,差异有统计学意义(χ~2=17.89,P<0.05);观察组的红肿消退时间(24.1±2.4)h、疼痛减轻时间(16.7±5.2)h、治愈时间(4.1±l.1)d,对照组红肿消退时间(38.3±4.1)h、疼痛减轻时间(26.8±4.1)h、治愈时间(6.7±1.9)d,观察组舒适度分值(95.0±4.3)、护理满意度分值(97.0±1.8),对照组舒适度分值(82.0±7.5)、护理满意度分值(90.0±2.3)。两组患者的红肿消退时间、疼痛减轻时间、治愈时间、舒适度分值、满意度分值均有统计学差异(P均<0.05)。结论水凝胶敷料联合复方七叶皂苷钠凝胶治疗PICC带管期机械性静脉炎效果优,患者无痛苦,操作简单易行,副作用少,疗效确切,患者易接受,可在临床推广使用。

Objective To observe the curative effect of hydrogel dressing combined with compound sodium aescinate gel in the nursing of mechanical phlebitis induced by PICC.Methods A total of 64 patients with mechanical phlebitis in tubal period of PICC who were admitted to our department from January 2013 to June 2016 were selected.The patients were divided into two groups：34 cases in the observation group,and 30 cases in the control group.Hydrogel dressing combined with compound sodium aescinate gel was used in the observatioin group from January 2015 to June2016.50%magnesium sulfate wet dressing was used in the control group from January 2013 to December 2014.The curative effect of mechanical phlebitis,swelling fading time,pain relief time,comfort score and nursing satisfaction were observed in the two groups.Results In the observation group,18 cases were cured within 7 days,9 cases were markedly effective,6 cases were effective and 1 case was ineffective.In the control group,4 cases were cured within 7days,5 cases were markedly effective,13 cases were effective,8 cases were ineffective,which were statistically significant（χ~2=17.89,P〈0.05）;the fading time of swelling was（24.1±2.4）h,the time of pain relief was（16.7±5.2）h,and the cure time was（4.1±1.1）d in the observation group.The fading time of swelling was（38.3±4.1）h,the time of pain relief was（26.8±4.1）h,and the cure time was（6.7±1.9）d in the control group.The comfort score was（95.0±4.3）,and nursing satis faction was（97.0±1.8） in the observation group.The comfort score was（82.0±7.5）in the control group,and the nursing satisfaction was（90.0±2.3） in the control group.There were statistically significant differences between the two groups in the fading time of swelling,pain relief time,cure time,comfort and satisfaction（P〈0.05 for all）.Conclusion Hydrogel dressing combined with compound sodium aescinate gel in the treatment of mechanical phlebitis during the tubal period of PICC has the advantages of favorable effect,no pain,simple operation,less side effect,definite curative effect,and being easily acceptable by the patients,which can be popularized clinically.
