某肿瘤专科医院PICC导管相关并发症影响因素分析

Analysis on influential factors of PICC catheter-related complications in a special cancer hospital

目的:对某三级甲等肿瘤专科医院在院PICC置管患者进行调查分析导管相关并发症发生的影响因素。方法:2015年8~9月调查在院PICC置管患者,记录置管及维护的情况,并观察其并发症发生的情况。结果:共调查190例置管患者,PICC置管主要应用在肿瘤科,PICC导管相关并发症发生例数为69例,导管相关并发症发生的危险因素为传统穿刺技术(盲穿)和未使用维护包进行维护,其中过敏性皮炎和导管脱出与未使用维护包有关,导管移位与盲穿技术有关。结论:随着PICC置管的增多和留管时间延长,其并发症发生率也相应增加;PICC置管普及B超引导下的穿刺技术(MST)或加强盲穿技术培训以及置管维护使用维护包,规范维护流程,加强护士维护技能的培训,可降低PICC相关并发症的发生。

Objective ：To investigate the PICC intubation patients in a 3A special cancer hospital to analyze the influential factor of catheter - related complica- tions. Methods：PICC intubation patients hospitalized in our hospital from August to September 2015 were investigated, the intuhation and maintenance condi- tions were recorded, and the occurring of complications was observed. Results：Totally 190 intubation patients were investigated, PICC intubation was mainly used in the tumor department, with 69 patients having catheter - related complications, the hazardous factor of catheter - related complications were the tradi- tional puncture technique （blind puncture） and the no use of maintenance bag for maintenance, in which the allergic dermatitis and loosening of catheter were related to the no use of maintenance bag, and the catheter displacement also was related with the blind puncture technique. Conclusion：With the increasing of PICC intubation and extension of catheter retention, the incidence of complications also increased accordingly; the popularization of PICC intubation and punc- ture technique guided by B ultrasound or strengthening the training of blind puncture technique and catheter maintenance, using maintenance bag, regulating the maintenance process and strengthening the nurses＇ maintenance skill training could reduce the occurring of PICC - related complications.
