颈椎间盘穿刺定位引导器的临床应用价值

THE CLINICAL VALUE OF APPLYING APPLYING POSITIONING DEVICE IN CERVICAL INTERVERTEBRAL DISC PUNCTURE

目的探讨颈椎间盘穿刺定位引导器的研制并评价其在颈椎间盘穿刺治疗中的作用和优势。方法纳入在临沂市中医医院住院的颈椎间盘突出症患者40例,随机分定位器组和指压组两组。定位器组在避免C型臂X光机照射下,利用颈椎间盘穿刺定位引导器行颈椎间盘定位引导穿刺治疗;指压组操作者直接在C型臂X光机照射下,通过指压法行颈椎间盘穿刺治疗,对比分析两组穿刺的成功率、不良反应及临床效果,以评价颈椎间盘定位引导穿刺器的临床应用价值。结果定位器组穿刺成功率优于指压组(P=0.003),且不良反应少于指压组(P=0.022)。指压法穿刺治疗有效率为70%,定位器法穿刺有效率为85%,两者比较定位器法穿刺有效率优于指压法(P=0.039)。结论 C臂下颈椎间盘穿刺定位引导器穿刺成功率高、临床效果好,且不良反应少,可以完全代替指压法行颈椎间盘穿刺治疗,值得在临床治疗中推广应用。

Objective To evaluate clinical value of applying positioning device in cervical intervertebral disc puncture.Methods Forty cervical disc herniation patients treated in Linyi City Traditional Chinese Medicine Hospital were randomly divided into two groups.The locator group received puncture without exposure to C-arm X-ray under the guidance of positioning device while the routine group received shiatsu with exposure to C-arm X-ray.The clinical effects were evaluated.Results Compared with the routine group,the locator group had higher success rate（P =0.003）and less adverse reactions（P =0.022）.The effective rate was 70%in the shiatsu group and 85%in the locator group（P =0.039）.Conclusion Applying positioning device in cervical intervertebral disc puncture can produce good clinical effect.
