子宫肌瘤孕妇在剖宫产同时行子宫肌瘤剔除术的临床分析

A clinical analysis of cesarean delivery and myomectomy at the same time for pregnant women with uterine fibroids

目的探讨合并有子宫肌瘤孕妇剖宫产时行子宫肌瘤剔除术的临床疗效。方法选取2014年2月至2016年4月我院收治的剖宫产术合并有子宫肌瘤的孕妇78例,其中38例在剖宫产同时行子宫肌瘤剔除术(联合组),另40例孕妇仅进单纯行剖宫产术(单纯组),比较两组患者手术时间、术中出血量、住院时间及术后并发症发生率。结果联合组患者手术时间长于单纯组,产后出血及产褥病发生率低于单纯组,差异有统计学意义(P<0.05);两组术中出血量、住院时间及切口感染发生率比较差异无统计学意义(P>0.05)。结论合并子宫肌瘤孕妇行剖宫产术时行子宫肌瘤剔除术虽然延长了手术时间,但不增加术后产后出血和产褥病发生率,安全性高,疗效显著。

Objective To investigate the clinical effect of cesarean delivery and myomectomy at the same time for pregnant women with uterine fibroids .Methods From February 2014 to April 2016 ,we performed the cesarean section for 78 pregnant women with uterine fibroids.Of the 78 patients,38 underwent myomectomy at the same time （combined group）.The rest 40 cases were taken as simple group .The operation time ,intraoperative bleeding ,hospitalization time and incidence of postoperative complications were com-pared between the two groups .Results The average operation time in the combined group was longer than that of the simple group ,but the incidence of postpartum hemorrhage and puerperal morbidity in the combined group was lower than that of the simple group .There was no significant difference in the average blood loss ,average hospitalization time and the incidence of incision infection between two groups （ P 〉0.05 ） .Conclusion Although cesarean delivery combined with myomectomy for hysteromyoma pregnant women prolong the operation time ,it does not increase the incidence of postpartum hemorrhage and puerperal morbidity .Therefore ,the procedure is safe and has high curative effect .
