金霉素眼膏在阻塞性睡眠呼吸暂停低通气综合征手术创面中的应用效果观察

Observation of application effects of chlortetracycline eye ointment on the surgical wound of obstructive sleep apnea and hypopnea syndrome

[目的]观察金霉素眼膏在阻塞性睡眠呼吸暂停低通气综合征(OSAHS)手术创面中的应用效果.[方法]选择96例OSAHS患者,随机分为对照组(46例)和观察组(50例),两组均施行悬雍垂腭咽成形术(UPPP),观察组术中将金霉素眼膏涂布于手术创面后进行缝合,而对照组术中手术创面内未涂布任何药物后直接进行缝合,观察两组术后出血发生率、出血部位及程度、手术创面水肿程度、术后瘢痕形成程度及术后缝合线情况.[结果]观察组和对照组中术后出血率分别为6.5%和22.0%,相比较差异有统计学意义(P<0.05).观察组扁桃体下极出血2例,弥漫性出血1例;对照组扁桃体窝出血4例,扁桃体下极出血4例,弥漫性出血1例,其他出血2例.观察组Ⅰ度出血3例;对照组Ⅰ度出血3例,Ⅱ度出血5例,Ⅲ度出血3例.观察组手术创面水肿程度及瘢痕形成程度较对照组明显减轻(P<0.01).[结论]在OSAHS手术创面涂布给予金霉素眼膏后进行缝合的方法可有效减少术后出血,明显减轻出血及手术创面水肿程度,减轻手术创面术后瘢痕的形成.

OBJECTIVETo observe the application effects of chlortetracycline eye ointment on the surgical wound of obstructive sleep apnea and hypopnea syndrome（OSAHS）.METHODS96 patients with obstructive sleep apnea and hypopnea syndrome were randomly divided into control group（46cases）and observation group（50cases）,both of them were taken uvulopalatopharyngoplasty（UPPP）.The surgical wounds of the patients in the observation group were daubed with chlortetracycline eye ointment,and then they were sutured.No drug was daubed on the surgical wounds of the patients in the control group,they were directly sutured.Postoperative bleeding rate,the bleeding site and degree,the degree of surgical wound edema,the forming degree of postoperative scarring and postoperative sutures were observed after the operation in both groups.RESULTS Postoperative bleeding rate in the observation group and the control group was 6.5% and 22.0% respectively,the difference had statistical significance（P〈0.05）.There were 2cases with the hemorrhage under the tonsil and 1case with the diffuse hemorrhage in the observation group.In the control group,there were 4cases with tonsillar fossa hemorrhage,4cases with hemorrhage under the tonsil,1case with diffuse hemorrhage and 2cases with other hemorrhage.3cases in the observation group hadⅠdegree of hemorrhage.In the control group,3cases hadⅠdegree of hemorrhage,5cases had Ⅱdegree of hemorrhage,3 cases hadⅢdegree of hemorrhage.The degree of surgical wound edema and postoperative scarring in the observation group was significantly lower than that in the control group（P〈0.01）.CONCLUSION Surgical wound suture combined with the use of chlortetracycline eye ointment could effectively decrease postoperative hemorrhage,the degree of hemorrhage and the surgical wound edema,and the formation of postoperative scarring.
