腹腔镜外科基础培训在住院医师规范化培训中开展的可行性分析

The feasibility analysis of basic training program of laparoscopic surgery in surgical residents in Shanghai normative training program of residents

腹腔镜外科基础培训在我国尚无标准化的培训与考核认证。采用美国腹腔镜外科基础的培训体系,对45名第一年外科住院医师及32名第四年外科住院医师进行为期8周的培训,并采用该培训规定的五项基本操作评估体系,对两组医师培训前后的得分进行比较。结果显示培训前PGY4组的得分显著高于PGY1组,但均未能达到合格标准,通过培训两组的得分均有显著提高,且能达到合格标准,但两组间得分并无显著差异。腹腔镜外科基础培训能使外科住院医师有效地提高腹腔镜操作技能。

There is no standard training program and testing system of laparoscopic surgery for surgical residents in China. Our retrospective study enrolled 45 junior surgical residents（ first year postgraduate,PGY1） and 32 senior surgical residents（ fourth year postgraduate,PGY4） who underwent fundamentals of laparoscopic surgery（ FLS） training program within the same 8-week period. Mean task scores and overall scores for two groups of surgical residents were compared before and after training respectively. Before the training,the overall normalized scores showed significant difference between PGY1 group and PGY4 group. But no one reached the qualified score. After the training,the scores in both groups raised,while there was no statistically significant difference between the two groups.The FLS program was useful in identifying technical and cognitive deficiencies,motivating surgeons to practice,and fostering significant improvements in skills.
