音乐运动疗法对脑卒中患者的运动功能、步行能力及心理的影响 被引量:53

摘要 目的:研究音乐运动疗法对脑卒中患者运动功能、步行能力及心理的影响。方法:32例脑卒中患者随机分入音乐运动疗法组和运动疗法组。运动疗法组接受为期3周的常规运动康复训练。音乐运动疗法组在接受常规运动康复训练的同时,配合聆听患者喜爱的音乐,共3周。分别采用Fugl-Meyer运动功能评分(FMA)量表、Holden步行能力分级、Borg量表、抑郁自评量表(SDS)和焦虑自评量表(SAS)评价运动功能、步行能力、自觉疲劳程度和神经心理状况。结果:两组患者治疗后FMA评分均较治疗前显著增高(P<0.05)。音乐运动疗法组治疗前后FMA差值显著高于运动疗法组(P<0.05)。音乐运动疗法组患者治疗后Holden步行能力分级显著高于运动疗法组(P<0.05),步行200m之后的Borg评分显著低于运动疗法组(P<0.05)。音乐运动疗法组患者治疗后SDS评分和SAS评分均显著低于治疗前评分(P<0.05),也显著低于运动疗法组治疗后评分(P<0.05)。结论:音乐运动疗法可提高脑卒中患者的运动功能和步行能力,减轻患者运动后的主观疲劳感觉,缓解患者的焦虑和抑郁症状。 Objective: To explore the effects of music-movement therapy on motor function, walking ability and psychology in stroke patients. Method: Thirty-two stroke patients were randomly divided into music-movement therapy group (MMT group) and exercise therapy group (ET group). Patients in ET group received conventional exercise rehabilitation for three weeks. Patients in MMT group listened to their favorite music while they were being received conventional exercise rehabilitation for three weeks. Fugl-Meyer assessment (FMA), Holden walking ability classification, Borg scale, self-rating depression scale and self-rating anxiety scale were used to assess movement function, walking ability, rating of perceived exertion, depression and anxiety. Result: The values of FMA after treatment in both groups were significantly higher than those before treatment (P<0.05). FMA difference before and after therapy in MMT group was significantly higher than FMA difference in ET group (P<0.05). Holden walking ability classification after therapy in MMT group was obviously higher than that in ET group (P<0.05). Borg score after 200 meters walking in MMT group was obviously lower than that in ET group (P<0.05). SDS and SAS after therapy in MMT group, which were obviously lower than those before therapy, were significantly lower than those in ET group (P<0.05). Conclusion: Music-movement therapy can improve the motor function and walking ability, reduce the subjective feeling of fatigue after exercise, and relieve anxiety and depression symptoms in patients with stroke.
