营养支持辅助治疗肺结核合并呼吸衰竭的疗效观察

目的通过观察营养支持辅助治疗肺结核合并呼吸衰竭的临床疗效，建立肺结核合并呼吸衰竭营养支持治疗的方法。方法将70例肺结核合并呼吸衰竭患者随机均分为对照组和观察组。对照组采用常规药物治疗，观察组采用常规药物辅助营养支持治疗，治疗前和治疗2周后比较两组患者体质指数、白蛋白、前白蛋白、总淋巴细胞计数与血气指标；并记录上述患者治疗过程中的副反应发生情况。结果观察组患者治疗2周后体质指数、白蛋白、前白蛋白与总淋巴细胞计数显著改善（P〈0．05）。且显著优于对照组患者（P〈0．05）；两组患者治疗后PaO2与PaCO2均得到显著改善（P〈0．05），且观察组患者改善效果优于对照组患者（P〈0．05）；治疗过程中两组患者出现的副反应病例经对症治疗后均好转，不影响正常治疗。结论在肺结核合并呼吸衰竭患者的临床治疗过程中，应积极采取辅助营养支持治疗措施，可显著提高临床治疗效果，是一种重要的临床辅助治疗方法。

Objective To observe the clinical effect of nutritional support adjuvant treatment in pulmonary tuberculosis patients with respiratory failure and set up the nutritional support adjuvant treatment methods. Methods 70 pulmonary tuberculosis patients with respiratory failure were randomly divided into control group and observation group, the control group was treated by conventional drug, observation group was treated by conventional drug and nutritional support adjuvant, the body mass index, albumin, prealbumin, total lymphocyte count and blood gas analysis were compared before and after treatment, recorded the side reactions during the treatment in the patients. Results The body mass index, albumin, prealbumin and total lymphocyte count were significantly improved （ P〈0.05 ） in the observation group after 2 weeks＇ treatment and significantly better than the control group＇ s （ P〈0.05 ） ; after treatment PaO2 and PaCO2 were significantly improved （ P〈0.05 ） in two groups and significantly better than the control group＇ s （ P〈0.05 ） ; the side reactions during the treatment in the patients were improved after symptomatic treatment and did not affect the normal treatment. Conclusions During treating the pulmonary tuberculosis patients with suction failure, nutritional support and adjuvant treatment should be applied actively which can significantly improve the clinical effect.
