动脉优先处理行保留脾脏腹腔镜胰体尾切除术15例

目的探讨“动脉优先”处理理念在腹腔镜保留脾脏胰体尾切除术中的应用。方法回顾总结15例应用“动脉优先”处理理念行保留脾动静脉腹腔镜胰体尾切除术的临床资料，探讨动脉优先处理在保留脾动静脉腹腔镜胰体尾切除术中的应用价值。结果15例患者均顺利完成腹腔镜下保留脾脏胰体尾切除，平均手术时间（96±44）min，术中出血量（82±26）ml，术后平均住院时间6．7d。术后未发生胰漏、出血、感染等并发症。结论动脉优先处理是保留脾动静脉腹腔镜胰体尾切除术中安全快捷的手术方法。

Objective To investigate the application value and clinical effect of the artery-first approach in the laparoscopic spleen and splenic vessels-preserving distal panereatomy. Methods To summarize the clinical data of 15 cases for splenic artery and vein laparoscopic pancreatic body and tail resection, and to investigate the application value and clinical effect of artery-first approach in the laparoscopic spleen and splenic vessels-preserving distal pancreatomy. Results All the 15 patients were operated successfully with an average operative time of （ 96 ± 44 ） min, blood loss （ 82 ± 26 ）ml and average duration of hospitalization was 6.7d.Postoperative pancreatic leakage, bleeding, infection and other complications didn＇ t occur. Conclusions Arterial preferential approach is a safe and efficient surgical approach of the artery-first approach in the laparoscopic spleen and splenic vessels-preserving distal pancreatomy.
