益肾降浊法配合结肠透析治疗慢性肾衰竭疗效观察

Curative effect of Yishen Jiangzhuo therapy combined with colon dialysis on chronic renal failure

目的观察益肾降浊法配合结肠透析治疗慢性肾衰竭的临床疗效。方法将70例慢性肾衰竭患者随机分为观察组和对照组。2组均给予常规治疗及结肠透析治疗,观察组在此基础上加服益肾降浊中药,以7 d为1个疗程,共治疗4个疗程。观察2组治疗前后肾功能指标、尿微量白蛋白、尿β_2-微球蛋白、血清炎性物质以及免疫指标变化情况。结果 2组治疗后SCr、BUN、24 h Upro、ALB、β_2-MG及血清hs-CRP、TNF-α、IL-6均显著降低(P均<0.05),Ccr均显著升高(P均<0.05),且观察组治疗后各指标改善情况均显著优于对照组(P均<0.05);治疗后,观察组CD4^+、CD4^+/CD8^+及LgM、LgG、LgA均显著升高(P均<0.05),CD8^+显著降低(P<0.05),对照组各项免疫指标均无明显变化(P均>0.05);观察组治疗后总有效率显著高于对照组(P<0.05)。结论益肾降浊法配合结肠透析能有效改善慢性肾衰竭患者的临床症状,减轻患者体内微炎症状态,增强患者免疫功能,提高治疗效果,值得临床推广应用。

Objective It is to observe the clinical curative effect of ishen Jiangzhuo therapy combined with colon dialysis on chronic renal failure. Methods 70 cases of patients with chronic renal failure were randomly divided into observation group and control group. Both groups were given conventional treatment and colon dialysis therapy, the observation group was given Yishen Jiangzhuo Chinese medicine on the basis of conventional treatment. 7 days as one treatment course, both groups were treated for 4 courses. The changes of renal function indexes, mieroalbminurine,β2-microglobulin, serum inflammatory substances and immune index before and after treatment in both groups were observed. Results After treatment, the levels of SCr, BUN, 24 hUpro, ALB, β2-MG and serum hs-CRP, TNF alpha, IL-6 were significantly decreased （P 〈 0.05） , that of Ccr was significantly increased （P 〈 0.05 ） , and the improvements of these indexes in observation group after treatment were significantly better than that of control group （P 〈 0. 05）. After treatment, the levels of CD4^＋ , CD4^＋/CD8^＋ and LgM, LgG, LgA were significantly increased （ P 〈 0. 05 ） , and that of CD8^＋ decreased significantly （ P 〈 0.05 ） , but no obvious change of these indexes was found in the control group （ P 〉 0. 05）. The total effective rate was significantly higher in observation group than that in control group after treatment （ P 〈 0.05 ）. Conclusion Yishen Jiangzhuo therapy combined with colon dialysis can effectively improve the clinical symptoms, relieve the patient＇ s body slightly inflammatory state, enhance immune function, improve the therapeutic effect in patients with chronic renal failure, which is worthy of clinical popularization and application.
