卡前列素氨丁三醇注射液治疗宫缩乏力性产后出血的疗效观察

The effect of Carboprost Tromethamine injection in treatment of postpartum hemorrhage

目的 观察卡前列素氨丁三醇注射液治疗宫缩乏力性产后出血的疗效。方法选择2014年12月~2015年12月在我院住院存在宫缩乏力因素的孕妇56例,所有产妇根据用药不同随机分为研究组和对照组,每组28例,两组均行腹壁横切口的子宫下段剖宫产术。对照组予缩宫素与米索前列醇,研究组患者予缩宫素、卡前列素氨丁三醇,比较两组治疗后的临床疗效,以及两组产后24h出血量及不良反应情况。结果研究组治疗后的的总有效率96.5%,无效1例,明显高于对照组治疗后的的总有效率75.0%,两组总有效率比较,差异存在显著性(P<0.05)。研究组产后24 h的出血量为(723.7±63.2)mL,对照组产后24 h的出血量为(964.6±58.2)mL,研究组产后24 h出血量显著少于对照组,差异存在显著性(P<0.05)。对照组出现恶心呕吐4例、血压升高8例,腹泻4例,研究组出现恶心呕吐2例,血压升高4例,腹泻3例,余未见明显不良反应。结论卡前列素氨丁三醇注射液治疗宫缩乏力性产后出血疗效显著,出血量少、安全性好。

Objective To observe the curative effect of Carboprost tromethamine injection in treatment of postpartum hemorrhage caused by uterine inertia. Methods From December 2014 to December 2015 in our hospital, 56 pregnant women were randomly divided into study group and control group according to different medication, 28 cases in each group, the two groups ＂underwent abdominal transverse incision of lower uterine segment cesarean section. The control group was given oxytocin and misoprostol, the study group was treated with oxytocin, oxytocin three alcohol, the clin- ical efficacy of the two groups after treatment were compared, postpartum 24 h bleeding and adverse reactions were compared between two groups. Results The total effective rate of the observation group after treatment was 96.5%, 1 case was ineffective,was significantly higher than that of the control groups 75.0% after treatment,the difference was significant between two groups （P〈0.05）. The amount of postpartum hemorrhage of 24 h in the study group was （723.7± 63.2） mL, the control group was （964.6±58.2） mL, the amount of 24 h in the study group was significantly less than that in the control group, the difference was significant （P〈0.05）. In control group, 4 cases of nausea and vomiting, 8 cases of high blood pressure, 4 cases of diarrhea, 2 cases of nausea and vomiting in the study group, 4 cases of hy- pertension, 3 cases of diarrhea, others had no obvious adverse reactions. Conclusion Carboprost tromethamine injec- tion in treatment of uterine atony postpartum hemorrhage curative is effect, has less bleeding, and it has good safety.
