摘要
目的:了解家庭医生制度的实施对居民社区门诊就诊状况的影响情况。方法:采集2012—2015年上海市虹口区某社区卫生服务中心的所有门诊数据,包括每名签约居民和未签约居民的基本信息和医疗费用、就诊次数等信息。结果:2012—2015年期间社区居民在该社区卫生服务中心门诊发生的就诊数据呈现增长趋势,其中签约居民在年就诊人数、年医疗总费用、年人均医疗费用、年人均就诊次数方面的增长率分别为137.87%、203.02%、27.95%和31.40%,均高于未签约居民的-18.55%、-32.46%、-15.25%和-21.69%。结论:家庭医生制度实施后,签约居民对社区卫生服务中心的服务利用水平显著上升,该项制度对引导居民就诊"下沉"社区的作用,值得深入研究。
Objective: To understand the impact of the implementation of family doctor system on community outpatient services. Methods: Outpatient service data from one community health service center in Hongkou District between 2012 and 2015 were collected, including basic information of each resident( whether signed with a family doctor or not ), number of outpatient visits, medical expenses and other information. Results: From 2012 to 2015, the residents' community outpatient services showed an increasing trend. The growth rate of number of people seeking outpatient services, annual total medical expenses, annual per capita medical expenses, annual per capita outpatient visits of the signed residents were 137.87%, 203.02%, 27.95% and 31.40% respectively, which were higher than those of unsigned residents ( -18.55%, -32.46%, -15.25% and -21.69% ). Conclusion: Since the implementation of the family doctor system, the utilization of community health services of signed residents rose significantly, suggesting that the system has played a role to encourage residents to seek health services in the community, and is worth further exploration.
