浦东八家社区卫生服务中心全科医师岗位培训现状及需求

Present situation and demands for on-the-job training of general practitioners in eight community health service centers in Pudong

目的:了解浦东新区陆家嘴医疗联合体(以下简称"医联体")内社区卫生服务中心现有全科医师的构成、教育培训经历、知识与技能掌握和需求情况,为社区卫生服务中心医疗服务队伍建设提供参考和依据。方法:对陆家嘴医联体内8家社区卫生服务中心(以下简称"社区")的80名职称为主治医师及以上的全科医师进行问卷调查。结果:社区全科医师女性多于男性,男女比例为1∶2.2;平均年龄(35.8±4.4)岁;93.8%的人学历为本科。78.8%的人曾得到过系统的全科医学教育,以毕业后继续教育和岗位培训为主。56.3%的人参加工作后得到过学历提升教育。曾参加全科医师岗位培训的人占75.0%,他们认为岗位培训对工作有提高作用,但存在重点不突出和过于形式化的问题。全科医师认为自身对基本技能和临床操作知识的掌握程度低于对基本理论的掌握程度,对基本技能和临床操作知识的培训需求大于对基本理论的培训需求。结论:应尽快建立社区全科医师定期轮训制度,制定出一套个性化和实用性强、可行性高的培训方案,定期选派全科医师到全科住院医师规范化培训基地培训,通过基地的协同和带动作用,最终实现社区全科医师和基层医疗的与时俱进和可持续发展。

Objective： To understand the composition, education and training experience, knowledge and skills of general practice, and demands for on-the-job training of general practitioners （ GPs ） in community health service centers affiliated with the health alliance of Lujiazui Area of Pudong, so as to provide references for human resource construction of community health service centers. Methods： Eighty general practitioners in eight community health service centers （ hereinafter referred to as community ） affiliated with the health alliance of Lujiazui Area were surveyed, who were with the professional title of attending physicians or above. Results ： There were more female GPs than male ; the ratio of male to female was 1 ： 2.2. The average age was （ 35.8 ± 4.4 ）years old. A total of 93.8% of the GPs had bachelor degree ; 78.8% of them received systematic general practice education, which was mainly post-graduation continuing education and on-the-job training. Among them, 56.3% received academic education; 75.0% participated on-the-job training of GPs and believed that the training was useful but formalistic, and the key contents were not highlighted. The investigated GPs believed that they mastered basic skills and clinical operation better than basic theory, and they had greater training demands for basic skills and clinical operation than basic theory. Conclusion： It is suggested to establish a regular rotation system for community GPs as soon as possible, develop a set of personalized, practicable and feasible training plan, and regularly send community GPs to the base to receive the standardized training for general practice, so as to sustainably improve the quality of community GPs and community health services.
