食管癌放化疗患者社会支持和生命质量对自我感受负担影响的路径分析

Path analysis on the effects of social support and QOL on SPB in esophageal neoplasms patients with ;chemoradiotherapy

目的：探讨食管癌放化疗患者社会支持、生命质量对自我感受负担的影响路径及关系效应。方法采用自我感受负担量表、社会支持评定量表和生命质量核心量表对130例食管癌放化疗患者进行测评,并采用Amos 7.0软件进行路径分析。结果患者自我感受负担、社会支持和生命质量总分分别为（59.60±11.42）分、（34.43±5.05）分和（50.32±16.82）分。上述三变量间存在相关性（r=0.411-0．592,P〈0.05）。社会支持对自我感受负担有直接影响,并通过生命质量对自我感受负担产生间接影响,总效应值为1.37。生命质量对自我感受负担有直接保护作用,亦是社会支持与自我感受负担的中介变量,总效应值为0.93。结论食管癌放化疗患者自我感受负担、社会支持和生命质量水平均不理想。自我感受负担受到社会支持和生命质量的直接、间接影响,可从各环节制定干预措施,以改善患者的自我感受负担。

Objective To explore the influence path and relationship effect of social support and quality of life （ QOL） on self-perceived burden （ SPB） in esophageal neoplasms patients with chemoradiotherapy. Methods A total of 130 patients were recruited into our study. Data were collected by self-perceived scale for cancer patients, social supporting rating scale and European organization for research and treatment of cancer quality of life questionnaire （EORTC QLQ）. Analysis of moment structures （AMOS） version 7.0 software was used to conduct the path analysis. Results The total scores of patients′ SPB, social support and QOL were （59.60±11. 42）, （34.43±5.05） and （50.32±16.82）, and there was correlation among them （r=0.411-0.592, P〈0.05） . Social support had direct effects on SPB and affected SPB indirectly via QOL. The total effect value was 1.37. QOL was a direct protective factor for SPB and it was also intermediary variable of social support and SPB. The total effect value was 0.93. Conclusions The level of SPB, social support and QOL in esophageal neoplasms patients are low. SPB are directly affected by social support and QOL. Interventions developed on the basis of influencing factors of SPB should be used to improve patients′SPB situation.
