胆囊切除术并发症的危险因素分析

Analysis of Risk Factors of Cholecystotomy Complications

目的探讨胆囊切除术相关并发症的危险因素。方法回顾性分析2013年1月—2016年1月江苏省常州市新北区孟河中医医院60例行胆囊切除术出现并发症患者的临床资料,观察并发症发生情况,采用单因素与多因素分析主要危险因素。结果年龄>45岁,病程>3年、结石是否多发对并发症发生率无显著影响(P>0.05),胆囊颈部结石(RR=1.195)、胆囊壁厚度(RR=0.326)、周围脏器粘连(RR=0.289)、Calot三角粘连(RR=0.274)为胆囊切除术并发症的主要危险因素(P<0.05)。结论胆囊颈部结石、胆囊壁厚度、周围脏器粘连、Calot三角粘连均为胆囊切除术并发症的主要危险因素。

Objective To discuss the risk factors of cholecystotomy complications. Methods The clinical data of 60 cases of patients with cholecystotomy treated in our hospital from January 2013 to January 2016 were retrospectively analyzed, and main risk factors were analyzed by the single-factor and multi-factor. Results The age more than 45, disease course more than 3 and stone had no obvious effect on the incidence rate of complications（P〉0.05）, and injury of biliary duct（RR=1.195）, gall-bladder wall thickness（RR=0.326）, peripheral organic adhesion（RR=0.289） and calot trigonometric adhesion（RR=0.274） were the main risk factors of cholecystotomy complications（P〈0.05）. Conclusion The injury of biliary duct, gall-bladder wall thickness, peripheral organic adhesion and calot trigonometric adhesion are the main risk factors of cholecystotomy complications.
