腹腔镜与传统手术治疗小儿腹股沟斜疝的对比研究

A Comparative Study Between Laparoscopic Surgery and Conventional Surgery in the Treatment of Pediatric Indirect Inguinal Hernia in Children

目的比较腹腔镜与传统手术治疗小儿腹股沟斜疝的临床疗效。方法根据患者家属的意愿将2014年1月~2015年5月我院收治的100例小儿腹股沟斜疝患儿分为2组:腹腔镜组(62例)和传统手术组(38例)。比较2组术中出血量、手术时间、住院时间,观察术后切口下出血和线结反应、皮下气肿、阴囊肿胀等并发症发生情况。结果传统手术组手术时间(24.3±6.5)min,与腹腔镜组(21.4±7.5)min无明显差异(t=1.972,P=0.051)。腹腔镜组出血量(6.8±2.7)ml,明显少于传统手术组(13.2±4.6)ml(t=8.777,P=0.000);住院时间(4.5±0.8)d明显短于传统手术组(6.9±1.2)d(t=12.002,P=0.000)。腹腔镜组和传统手术组并发症发生率无明显差异[3.2%(2/62)vs.7.9%(3/38),χ~2=0.322,P=0.571]。结论腹腔镜手术治疗小儿腹股沟斜疝在出血量、住院时间方面体现出微创技术的优势,在手术时间和术后并发症方面与传统手术差别不大,均能达到满意的临床效果。

Objective To compare the clincial therapeutic effects between laparoscopic surgery and conventional surgery in the treatment of pediatric indirect inguinal hernia in children. Methods According to the willing of family members,a total of 100 children with indirect inguinal hernia from January 2014 to May 2015 were divided into two groups： laparoscopic surgery group（ 62cases） and conventional surgery group（ 38 cases）. The amount of blood loss,duration of surgery,and length of hospital stay were compared between the two groups,and the complications such as incision bleeding,knotting reaction under incision,pneumoderm,and scrotum swelling were observed after surgery. Results There was no difference between the two groups in regard to the duration of surgery [（ 24. 3 ± 6. 5） min vs.（ 21. 4 ± 7. 5） min,t = 1. 972,P = 0. 051]. However,the blood loss was less in the laparoscopic surgery group than that in the conventional surgery group [（ 6. 8 ± 2. 7） ml vs.（ 13. 2 ± 4. 6） ml,t = 8. 777,P = 0. 000],and the length of hospital stay was shorter in the laparoscopic surgery group than that in the conventional surgery group [（ 4. 5 ± 0. 8） d vs.（ 6. 9 ± 1. 2） d,t = 12. 002,P = 0. 000]. There was no difference between the two groups in complications [3. 2%（ 2 /62） vs. 7. 9%（ 3 /38）,χ^2= 0. 322,P = 0. 571]. Conclusions Laparoscopic surgery presents advantages of minimal invasion in blood loss and length of hospital stay,and is similar to conventional surgery in the duration of surgery and complications. It can achieve satisfactory clinical effects in the treatment of pediatric indirect inguinal hernia.
