幽门螺杆菌检测在胃癌诊断中的临床意义

The clinical significance of Hp detection in the diagnosis of gastric cancer

[目的]探讨幽门螺杆菌(Hp)检测在胃癌诊断中的价值。[方法]选取2015年5月~2016年5月在我院接受胃镜检查并作病理活检患者372例,其中慢性浅表性胃炎104例,慢性萎缩性胃炎110例,不典型增生71例,胃癌患者87例。采用苏木精-伊红染色观察各患者胃黏膜病变组织Hp感染,采用Hp特异性抗体试剂检测患者血液抗体情况。[结果]各患者Hp检测阳性比较差异有统计学意义(P<0.05);随着病情的不断加重,苏木精-伊红染色和抗体检测阳性率增加,其中胃癌患者苏木精-伊红染色阳性和抗体检测阳性分别为80.46%和79.31%,明显高于其他患者(P<0.05);早期胃癌苏木精-伊红染色阳性和抗体检测阳性分别为61.54%和65.38%,均明显低于进展期胃癌患者,差异比较有统计学意义(P<0.05)。[结论]Hp感染在胃黏膜病变中起着重要的作用,及早诊断Hp感染并予以根除,对预防胃癌具有重要意义。

[Objective]To investigate value of Hp detection in the diagnosis of gastric cancer. [Methods] From May 2015 to May 2016,372 cases in our hospital who underwent gastroscopy and biopsy were selected,among whom there were 104 cases of chronic superficial gastritis, 110 cases of chronic atrophic gastri- tis,71 cases of atypical hyperplasia,and 87 cases of gastric cancer. HE staining was used to observe gastric mucosal lesions Hp infection. The blood antibody was detected with Hp specific antibody. [Results]There was statistically significant difference in Hp detection between each patient（P〈0.05）. With the aggrava- tion of the disease, the positive rate of HE staining and antibody detection was increased. HE staining and antibody positive were 80.46 % and 79.31% in gastric cancer patients,which were significantly higher than those in other patients（P〈0.05）. HE staining positive rate and antibody positive rate in early gastric canc- er were 61.54% and 65.38% ,which were significantly lower than the patients with advanced gastric cancer （P〈0.05）. [Conclusion]Hp infection plays an important role in gastric mucosal lesions. Early diagnosis of Hp infection and eradication is of great significance for the prevention of gastric cancer.
