颈后路单开门椎管成形术与全椎板减压术治疗长节段颈椎后纵韧带骨化症效果比较 被引量:8

摘要 目的比较颈后路单开门椎管成形术与全椎板减压术治疗长节段颈椎后纵韧带骨化症的临床效果。方法选择拟行手术治疗的长节段颈椎后纵韧带骨化症患者67例,随机分为成形术组36例和减压术组31例,分别行颈后路单开门椎管成形术(钛板法)和全椎板减压术(侧块螺钉法)。两组术前及术后24个月行日本骨科协会(JOA)评分及颈椎正侧位X线检查,计算JOA评分改善率及颈椎曲度指数(CCI);记录两组术后轴性症状评分及C_5神经根麻痹的发生率。结果两组术后JOA评分均高于术前(P均<0.05);两组术后JOA评分及其改善率比较差异均无统计学意义(P均>0.05)。两组术后CCI均低于术前,但成形术组降低更明显(P均<0.05)。成形术组术后CCI丢失度、术后轴性症状评分均高于减压术组,C_5神经根麻痹的发生率低于减压术组(P均<0.05)。结论颈后路单开门椎管成形术与全椎板减压术均可改善长节段颈椎后纵韧带骨化症患者的神经功能;前者术后轴性症状轻、C_5神经根麻痹发生率低,后者维持颈椎生理曲度效果好。
