专项处方点评对某三甲综合医院糖皮质激素合理应用的影响

Analysis of the Effects of Appropriative Prescription Evaluation on the Use of Glucocorticoid in Outpatient and Emergency Department of a Grade-A Tertiary General Hospital

目的探讨专项处方点评对某三甲医院糖皮质激素、门急诊处方规范化使用的影响,为推进其合理用药提供参考。方法利用医院信息管理系统,随机抽取专项处方点评前后各600张处方,分别从使用率、科室分布、疾病分布及不合理使用处方进行比较分析。结果耳鼻咽喉科及儿科分布率最高;耳鼻咽喉疾病及呼吸系统疾病使用率最高;专项点评实施后,糖皮质激素使用率由2.96%下降至1.93%,不适宜处方率由57.50%下降至17.00%,明显降低(P<0.05)。结论实施专项处方点评后,糖皮质激素处方使用率及不合理率明显降低,对保障用药安全、提高医师正确的用药意识具有积极意义。

Objective To explore the effects of the appropriative prescription evaluation on the rationality of glu- cocorticoids in Outpatient and Emergency Department of a Grade - A Tertiary General Hospital, so as to provide reference for the rational application of drugs in clinic. Methods By hospital information system （ HIS）, the outpatient and emergency prescriptions with glucocorticoid were randomly extracted 600 prescriptions from hospital before and after modified method of prescription comment. The utilization ratio, distribution of department and disease, irrationality of glucocorticoid was contras- tively analyzed. Results The distribution rate of Otolaryngology and pediatrics was the highest, and the incidence rate of O- torhinolaryngology diseases and respiratory diseases were the highest. Using the method of appropriative prescription evalua- tion, the utilization ratio of glucocorticoid decreased from 2. 96% to 1.93%, and the irrationality rate decreased from 57. 50% to 17. 00% （P 〈 0. 05 ）. Conclusion The appropriative prescription evaluation can significantly decrease the utili- zation rate and irrationality rate of glucocorticoids, which plays the active role in ensuring medication safety and improving the consciousness of use drug.
