彩超TEI指数对急性心肌梗死患者心功能的评估价值

Value of color Doppler ultrasound TEl index in evaluation of heart function in patients with acute myocardial infarction

目的探讨彩超心肌做功指数（TEI指数）对急性心肌梗死患者心功能的评估价值。方法选取2013年2月至2014年10月急性心肌梗死患者33例纳入观察组，同期体检的健康志愿者33例纳入对照组，均采取彩超及实验室检查，比较两组血清肌钙蛋白T（cTnT）、血浆N-末端脑钠肽前体（NT-proBNP）、左室射血分数（LVEF）及左心室TEI指数，并分析左室TEI指数与NT－proBNP、cTnT、LVEF的相关性。结果观察组TEI指数（0.48±0.14）、NT-proBNP[（351.24±3.81）ng/L]、cTnT[（2.65±0.33）μg/L]明显高于对照组[0.30±0.06、（100.06±4.26）ng/L、（0.32±0.17）μg/L，P〈0.05]，观察组LVEF[（48.19±1.06）%]低于对照组[（60.66±2.02）%，P〈0.05]；相关分析显示急性心肌梗死患者彩超TEI指数与NT-proBNP、cTnT呈正相关（γ＝0.754，0.698，P均〈0.05），与LVEF呈负相关（γ＝-0.731，P〈0.05），尚不能认为健康志愿者彩超TEI指数与NT-proBNP、cTnT、LVEF有相关性（γ＝0.233，0.174，-0.128，P均〉0.05）。结论彩超TEI指数对急性心肌梗死患者心功能具有较高的评估价值，值得在临床推广应用。

Objective To investigate the value of color Doppler ultrasound TEI index in evaluation of heart function in patients with acute myocardial infarction.Methods Thirty-three patients with acute myocardial infarction from February 2013 to October 2014 were included in the observation group, 33 healthy volunteers in the same period were included in the control group, color Doppler ultrasound and laboratory examination were adopted in the two groups, serum troponin T （cTnT）, plasma N-terminal pro-brain natriuretic peptide levels （NT-proBNP）, left ventricular ejection fraction （LVEF） and left ventricular TEI index in the two groups were compared, the relationship between left ventricular TEI index and NT-proBNP, cTnT and LVEF were analyzed.Results TEI index in the observation group （0.48±0.14）, NT-proBNP [（351.24±3.81）ng/L], cTnT [（2.65±0.33）μg/L] were significantly higher than those in the control group [0.30±0.06, （100.06±4.26）ng/L, （0.32±0.17）μg/L, P〈0.05], LVEF [（48.19±1.06）%] in the observation group was significantly lower than the control group [（60.66±2.02）%, P〈0.05]; correlation analysis showed that color Doppler ultrasound TEI index were positively correlated with NT-proBNP and cTnT in patients with acute myocardial infarction （γ=0.754, 0.698, P all〈0.05）, and were negatively correlated with LVEF （γ=-0.731, P〈0.05）; color Doppler ultrasound TEI index of healthy volunteers were not correlated with NT-proBNP, cTnT, LVEF （γ=0.233, 0.174, -0.128, all P〉0.05）.Conclusions Color Doppler ultrasound TEI index has high value in evaluation of heart function of patients with acute myocardial infarction, has value of clinic application.
