降钙素原及同型半胱氨酸与2型糖尿病合并冠心病患者冠状动脉病变的相关性 被引量:23

An association between procalcitonin,homocysteine and coronary artery disease in patients with type 2 diabetes mellitus complicated with coronary heart disease

摘要 目的:探讨降钙素原(PCT)及同型半胱氨酸(Hcy)与2型糖尿病合并冠心病患者冠状动脉(冠脉)病变的相关性。方法:入选121例2型糖尿病患者,分为单纯糖尿病组、糖尿病合并冠心病组(合并稳定型心绞痛组和合并急性冠脉综合征组),另取50例健康人为对照组。分析各组间一般临床资料,根据冠脉病变支数、Gensini评分及多元回归分析探讨糖尿病患者PCT及Hcy水平与冠脉病变的关系。结果:各组间Hcy、PCT水平比较,糖尿病合并稳定型心绞痛组、糖尿病合并急性冠脉综合征组显著高于健康对照组及单纯糖尿病组(均P<0.05);Hcy水平与PCT水平呈正相关关系(r=0.792,P<0.01);双支、三支冠脉病变组血清Hcy和PCT水平高于单支病变组(P<0.05);Logistic回归分析显示,Hcy、PCT、糖化血红蛋白是冠脉病变程度的独立危险因素。结论:2型糖尿病合并冠心病患者冠脉病变程度随血清Hcy和PCT水平增加而加重。 Objective:To investigate the effects of procalcitonin(PCT)and homocysteine(Hcy)on coronary artery disease in patients with type 2diabetes mellitus(T2DM)with coronary heart disease(CHD).Method:A total of 121 patients with T2 DM was divided into T2 DM group,T2 DM with CHD group(combined with stable angina group and acute coronary syndrome group),other 50 healthy cases were included into control group.The clinical data were collected,and the relationship between PCT and Hcy to coronary artery disease was analyzed according to the number of coronary lesions,Gensini score and multiple regression analysis.Result:The levels of Hcy and PCT in groups of T2 DM with stable angina and acute coronary syndrome were significantly higher than those in control group and T2 DM group(all P〈0.05).The level of Hcy was positively correlated with PCT level(r=0.792,P〈0.01).The levels of Hcy and PCT in double and three coronary lesion group were higher than that in single coronary lesion group(both P〈0.05).Logistic regression analysis showed that Hcy,PCT and glycosylated hemoglobin were independent risk factors of the severity of coronary artery lesions.Conclusion:The severity of coronary artery disease aggravate with the increase of Hcy and PCT levels in patients with T2 DM complicated with CHD.
