摘要
目的通过对北京市密云地区腹泻患者粪便标本监测结果的分析,了解腹泻致病菌的型别分布及耐药特征,为细菌性腹泻的治疗提供科学依据。方法 2014年1月-2016年9月,以密云区医院和密云中医院为哨点医院,收集腹泻患者粪便标本,分析腹泻致病菌的型别分布及耐药特征。结果共监测腹泻患者1 400例,分离致病菌123株,分离阳性率为8.8%,其中沙门菌56株,致泻性大肠埃希菌29株,副溶血性弧菌38株,未检出志贺菌。仅3株副溶血性弧菌对氨苄西林耐药。沙门菌和致泻性大肠埃希菌对喹诺酮类、四环素类耐药率较高。41.1%的沙门菌和44.8%的致泻性大肠埃希菌为多重耐药,这2种菌有耐药模式重叠情况。结论沙门菌和致泻性大肠埃希菌耐药情况比副溶血性弧菌严重,应加强致病菌耐药趋势的主动监测。
Objective To understand the distribution of bacteria types and durg resistance characteristics of diarrhea pathogens by analyzing the detection results of fecal specimens from diarrhea patients in Miyun, Beijing, so as to provide scientific basis for the treatment bacterial diarrhea. Methods Fecal specimens of hiarrhea patients were collected from sentinel hospitals, in- cluding Miyun Hospital and Miyun Hospital of Traditional Chinese Medicine from January 2014 to September 2016. And the dis- tribution of bacteria and characteristics of durg resistance were conducted for analysis. Results A total of 1 400 diarrhea pa- tients were monitored, and 123 strains of pathogens were detected, with positive rate as 8.8%. Among them, there were 56 strains of Salmonella, 29 strains of diarrheogenic Escherichia coli, 38 strains of Vibrio parahaemolyticus, but Shigella was not detected. Only 3 strains of Vibrio parahaemolyticus were resistant to ampicillin. The resistance rates of Salmonella and Esche- richia coli were higher to quinolones and tetracycline. 41.1% of Salmonella and 44.8% of diarrheogenie Escherichia coli were multidrng resistant, and the 2 kinds of bacteria had overlap on drug resistance patterns. Conclusion Salmonella and diarrheo- genic Escherichia coli had more severe resistance than Vibrio parahaemolyticus. It is necessary to strengthen the active monitoring in drug resistance trend.
