摘要
建立分级诊疗制度是医药卫生体制改革的重要内容之一。杭州市从实施布局、协作转诊流程、资源纵向流动、信息互联互通等方面顶层设计区域分级诊疗信息化体系,包括电子健康档案系统、区域卫生信息平台、远程会诊、全科医生签约、双向转诊、跨省转诊6个部分,通过信息互联互通把相应平台上的各类服务应用相互协同起来。运行3年以来,全科医生签约覆盖面逐年扩大,分级诊疗机制进一步建立,协作机制进一步健全,群众对改革有了获得感。
Setting up the hierarchical medical system is an important content of medical and health system reform. In the aspects of implementation layout, referral process coordination, longitudinal resource flow, and information connectivity, Hangzhou City developed a top-level design for regional information-based hierarchical medical system consisting of six parts, i.e., electronic health record system, regional health information platform, tele-consultation, general practitioner signing, dual referral, and transprovincial referral, to coordinate a variety of service applications on corresponding platforms by means of information connectivity. After three years of running, the coverage of general practitioner signing is being expanded year by year, and the hierarchical medical system has been further established and coordination mechanism further improved, making the masses of people feel a sense of gain from the reform.
